The treatment plan for #37 had already progressed to a consultation for extraction and immediate implant placement.
However, the words of the great masters of Perio and Endo—“You can always extract later; preserve the tooth whenever possible”—kept echoing in my mind.

I performed gingivectomy with an electrosurgical unit, removed the caries, built up a resin core, and prepared the tooth vertically. At the next visit, I will fabricate the provisional.
My intention in changing the plan was to keep the tooth functional with its periodontal ligament—whether for two years or five—through a nonsurgical, minimally invasive approach.

I fully understand the value of implants, but I never want to be bound or “cursed” by them. A dentist’s duty is to protect natural teeth and create an oral environment that can be maintained with ease. That’s easy to say, yet long-term preservation is challenging no matter which path you choose.

37はC4で抜歯即時埋入のコンサルまで決まってたんですが、「抜歯はいつでもできる　可及的に歯牙を保存しろ」と言ってたPerio/Endo界の重鎮達の言葉がリフレイン。電メスで歯肉切除し、カリエス除去し、レジンコア築造しVertical preparationしました。次回プロビジョナル作製。非外科的で2年でも5年でも歯根膜使って咬ませてあげられたらいいなと思っての、治療法転換。低侵襲で患者さんを喜ばせられる可能性があれば、そちらを選んで差し上げたい。

インプラントの良さも分かるけど、インプラントに呪われないように。歯科医師は歯を守り、メンテナンスしやすい口腔内をつくるのが職務。言うのは簡単だけど、長期保存はどちらも難しい。

I used the same sampling technique that performs.
We drilled a hole in the metal core large enough for the tip of a bridge remover to engage, then gently tapped and pulled it out. Naturally, we take great care to avoid root fracture, but even a metal core with virtually no ferrule like this could be removed successfully.

Applying an ultrasonic scaler to create micro-vibrations is also essential, as it helps disrupt the cement cohesion and gives you that extra “tailwind” during removal.Thx,Kaki Bro！

がやってたのサンプリング！メタルコアにブリッジリムーバー先端が嵌る程の穴を開けて、コンコン引っ張り上げるやつ。当然歯根破折にはかなり配慮しますが、こんだけフェルールのない大変なメタルコアも除去できました！超音波スケーラーを当てる事で、セメント凝集破壊の追い風吹かすのも大切だと思う。柿内くん、素敵な情報ありがと😊

Tooth 36 underwent perforation repair by Dr. Umeda (). For 37, a distal root fracture initially led us toward implant therapy, but because the patient works in healthcare and was highly cooperative, I proposed Plan B instead. Thank you, Dr. Umeda, as always.

We proceeded with autogenous tooth transplantation. Transplanting the large, curved-root 28 into 37 was challenging, yet the patient’s excellent plaque control allowed uneventful healing—2 mm pockets with no BOP—and we completed the case with a full-zirconia crown.

Bone was harvested under the 37 mesial pontic area with a trephine, ground, mixed 1:1 with graft material, and placed between the root and crestal bone.

Although more demanding than implant therapy, using a viable PDL is invaluable. This case reminded me that success depends heavily on patient selection; the right patient makes all the difference.

36は　梅田先生にパーフォレーションリペアしていただき、37遠心根破折は一度はインプラントにしようと決まっていましたが医療系職種で理解力あり協力的な患者さんであった為Plan Bを提案。梅田先生いつもありがとうございます。

今回は、自家歯牙移植に調整。歯牙サイズ大きく、弯曲根の28を37に移植するのはかなりチャレンジケースでした。しかし患者さんの丁寧なプラークコントロールのお陰で、全周ポケット2mmでBOPもなく順調な治癒を辿り、Full Zirconia Crownにて補綴できました。

37近心のポンティック下からトレフィンバーにて採骨し、ボーンミルで挽いて骨補填材と5:5で混合し、歯根と骨頂の間に忍ばせました。

インプラントより難しい移植ですが、やはり使える歯根膜は使いたいものです。しかし、患者さんのレベルによっては上手くいくものも大失敗となりえるのでケースセレクションがとても大切だと痛感。

My first-ever case using VISTA three times a week.
The reason for the gingival recession is presumed to be the excessive lateral load transferred to the premolar, caused by the congenital absence of tooth 13 and the subsequent attrition of the retained primary tooth.

I reshaped and polished the overly extended Class V composite restoration that spanned from the cervical area to the root surface, intentionally exposing the root surface before performing tunneling → suture bonding → and positioning the CTG.
Although the outcome may be temporary, I am also considering restoring the crown morphology of the primary tooth.

歯肉退縮の理由は13番先天欠如による乳歯晩期残存の摩耗による小臼歯への過度な側方力負担だと推測されます。
Class5の歯頚部から根面に渡る過剰なCR修復を、形態修正研磨して、根面を敢えて露出させてからトンネリング→スーチャーボンディング→CTGをポジショニングしました。一時的かもしれないが、乳歯の歯冠形態の修復も考えたいと思います。

A patient with severe bruxism ultimately suffered a vertical root fracture of the mandibular second molar (No.47). Due to the acute symptoms, the tooth was extracted immediately. After the patient prepared the implant budget, we proceeded with an early implant placement approximately 2months later.
A healing abutment (φ5.5 × 5.0) was selected, allowing us to position the implant platform 4 mm subcrestal from the buccal keratinized tissue. This also ensured that no redundant soft tissue would ride over the healing abutment, eliminating the need for unnecessary soft-tissue trimming.

The final restoration for tooth No.47 was fabricated using the 4S Concept without going through a PVR phase, and thanks to the excellent craftsmanship of .lazarus.inc, the case was completed flawlessly with no complications.

ブラキサーでとうとう歯根破折した47番。急性症状もありすぐに抜歯したが、インプラント費用をご用意いただき、約2ヶ月後にインプラント早期埋入。Healing Abutmentはφ5.5×5.0を使用する事で、頬側付着歯肉から4mm縁下にプラットフォームが位置するような埋入を確認できる。HA上に歯肉が、乗って治癒しないのも無駄な歯肉切除などの手間を省ける。

47番はPVRを経由せず4S Conceptの補綴形態でFinal Restorationを製作していただいたが、 .lazarus.inc の素晴らしい技工で何の問題もなく補綴完了した。いつもありがとうございます！

Even if the amount of gingival recession is small, if there is tooth sensitivity, we will perform aggressive root coverage treatment early. This time, I performed periodontal plastic surgery by smoothly inserting a CTG using the VISTA technic. The single incision on the left palate was the thickest and most uniform one I have ever taken. I am happy to feel that I am gradually improving my skills.

歯肉退縮量が少なくても知覚過敏がある場合は、早めに積極的な根面被覆治療を行います。今回もVISTA technicでCTGをスルっと入れて歯周形成外科を行いました。左口蓋のSingle Incisionは今までで一番厚く均一に採取できました。少しずつ技術が身についている実感がでてきて嬉しい。

A case that made me feel Full Zirconia Onlays are definitely a valid option.

This is Onlay, not Inlay.
These days I often go for overlays, but when the patient has stable periodontal conditions, a well-balanced occlusion, good comprehension, and excellent cooperation, an onlay is still a solid choice.

With this design, I felt lithium disilicate would carry a higher risk of fracture, so I selected Full-zirconia for this case.

今月はVISTA祭り！3Caseやる1Case目。

左口蓋からSingle IncisionでCTG採取するのめちゃくちゃ難しい🤨なんとか採取して、　.ono.dds にで教わった網タイツ縫合みたいなの初めてちゃんと出来ました。いつも頭こんがらがって失敗してたので術前にセミナー動画見返して良かった！記憶に定着させます。

同じ方の反体側でVISTAしたけど、バイオタイプは改善されたけど骨の裏打ちがあまりなかったからか割と下がってしまった反省を活かしてのオペでした。手技よりも診査診断の大切さを痛感しました😭今回こそ完全根面被覆できますように🙏
