ISHA - The Hip Preservation Society Our vision is to be the premier international society for education and research in hip preservation

ISHA – The Hip Preservation Society is an international organisation which is focused on hip preservation surgery to enhance patient care and outcomes, through the delivery of training, education, support of research, sharing of knowledge and best practice, fostering global collaboration and provision of patient and professional information to improve hip care. Our mission is to create “healthy hips” by bringing hip preservation, prevention and treatment to all people regardless of age, social status, gender or economic situation. Our vision is a world with equitable access to quality treatment and the best achievable patient outcomes through improved access to appropriate treatment, early intervention, prevention and education, with the goal of us living in a world where hip joint replacement is only an occurrence of old age. ISHA’s purpose is to advance, enhance, and raise awareness of the field of hip preservation, to facilitate early intervention to improve patient outcomes.

At the end of last year we issued a call for expressions of interest in joining ISHA committees and received the biggest response to date, with over 140 members putting their names forward!

The long process of sorting volunteers into appropriate committees, subcommittees and taskforces is now complete, and all members that expressed an interest should now have received an email indicating their allocated positions. Kick-off calls will be taking place over the coming weeks.

We’d like to recognise several long-standing members whose dedication to ISHA and to advancing excellence in hip preservation has spanned many years...

Dr Diana Silas is a hospital-affiliated orthopaedic surgeon practising in metro-Detroit within a multi-specialty orthopaedic group.

Having completed her sports medicine fellowship at the University of Rochester and dedicating a large portion of her fellowship to hip arthroscopy, Dr Silas’ practice is focused on arthroscopic procedures, with a large portion being hip arthroscopy.

Dr Silas is a long-term contributor to ISHA and recently joined the Editorial Taskforce within the Education Committee.

Title of Study: Prior hip arthroscopy impacts long-term outcomes of total hip arthroplasty : a propensity-matched study with a minimum ten-year follow-up

Long-term comparative analysis of patients who underwent total hip arthroplasty (THA) with a history of previous ipsilateral hip arthroscopy (PA) to a propensity-score matched control group of primary THA with no prior hip arthroscopy (NPA).

The PA group was propensity-score matched 1:1 based on age at THA, BMI, s*x, robotic assistance, approach, and laterality to the NPA group.

Clinical hip arthroplasty outcome thresholds, complications, and revision surgery rates were compared between cohorts. A Kaplan-Meier analysis was performed to assess survivorship.

A total of 108 patients were included, 54 in each group.

The groups displayed comparable outcomes at minimum ten-year follow-up, for mHHS (p = 0.370), HHS (p = 0.370), HOOS-JR (p = 0.380), FJS (p = 0.250), VAS (p = 0.150), and patient satisfaction (p = 0.310).

The PA group exhibited a significantly higher complication rate, with 11 major complications compared to two in the NPA group, translating to a relative risk of 2.8 (p < 0.033)

Conclusion
Patients undergoing primary THA with a history of prior hip arthroscopy achieve similar long-term functional outcomes compared to a propensity-matched control group. However, they face a 2.8-fold increased risk of complications and a 4.5-fold higher risk of major complications requiring revision THA.

The ISHA Research Committee is now accepting proposals for International Delphi Consensus studies for 2026.

Two successful studies will be supported annually by the Delphi sub-committee (study design, expert panel selection, survey construction, data collection and statistical analysis), with the intention of peer-reviewed journal publication following study completion.

If you have a research idea which is suitable for a Delphi Consensus study process, please consider submitting it for consideration.

All submissions must adhere to the ISHA Delphi Study application requirements which can be viewed via this link.
