In certain instances, despite accurate determination of tooth length via X-ray and Apex Locator, gutta percha extrusion beyond the apex occurs upon filling.

What is the underlying cause?

One primary reason is the alteration in working length following canal shaping. Specifically, curved canals or those with a dentinal triangle exhibit a 0.5 to 1 mm decrease in length post-shaping, whereas straight canals with adequate access remain unchanged. To ensure precision, reevaluation using the Apex Locator is essential prior to filling. The decreased length is attributed to the initial winding and obstructed path taken by the file, which becomes straighter and shorter after shaping, illustrating that a straight path is invariably shorter than a curved one.
