ENT Signs Suggestive of GERD / LPR

Laryngeal & Pharyngeal Findings

• Posterior commissure hypertrophy (“pachydermia” at interarytenoid region)

• Arytenoid erythema and edema (especially medial surfaces)

• Vocal fold edema (often diffuse, not focal like nodules)

• Pseudosulcus (subglottic edema giving an apparent “double edge” of the vocal folds)

• Thick, sticky endolaryngeal mucus coating the folds

• Contact ulcers / granulomas on the vocal process of arytenoids

• Posterior pharyngeal wall cobblestoning or erythema =lymphoid follicles

Extra-laryngeal / Indirect Signs

• Nasopharyngeal inflammation or adenoid hypertrophy in children

• Eustachian tube edema contributing to middle ear effusion

• Chronic throat clearing with no obvious infection

• Halitosis and thick postnasal secretions not explained by rhinitis

Important Points

• These signs are not pathognomonic — many overlap with allergy, voice abuse, infection.

• They should always be interpreted together with history:

◦ Globus sensation

◦ Chronic cough (esp. nocturnal)

◦ Hoarseness, worse in the morning

◦ Dysphagia or throat burning
