People with disabilities often face significant barriers in accessing palliative care♿, due to poor coordination between disability and healthcare services. This can lead to hospital transfers and spend the end-of life in unfamiliar settings rather than at home👨‍👩‍👧‍👦. In this blog, Lindsey Moore and Mariana Ricci from SA Health highlights the “A Respectful Death at Home” project by Minda and Palliative Care Connect, which aims to improve end-of-life care for people with disability. The project builds staff knowledge, strengthen collaboration, and improves care pathways. 💛
