I want to thank all the clients that have chosen me to care for them and walk along side them for their journey’s. It is a privilege I don’t take for granted.

Is the recommendation of induction for advanced maternal age truly based on sound evidence?

Or is it illogical when you break down and really look at the issues?

What do older women need to know about the evidence in this area?

We hear from more and more women who are told that they need induction of labour because they are older.

This issue also arose when the NICE guideline on induction of labour was revised in 2021.

The recommendation to offer earlier induction to older women was not included in the final guideline.

But we know it’s still being offered.

As I wrote when the guideline was published:

“One of the most controversial aspects of the draft [NICE] guideline was the proposed recommendation that earlier induction be offered to certain groups of women, including older women, larger women and women who conceived via IVF or ARTs.

It is clear that some groups of women/babies have a slightly higher chance of stillbirth compared to other groups.

But data on this are often crude, the absolute risk may not be that high and we often have no trial evidence to show whether or not induction of labour would make a difference.” (Wickham 2021).

I’ve written more about induction for advanced maternal age in two of my books.

Those are Inducing Labour: making informed decisions and In Your Own Time: how western medicine controls that start of labour and why this needs to stop.

I also have a blog post on induction for advanced maternal age.

Find it at https://www.sarawickham.com/articles-2/induction-for-advanced-maternal-age/

I hope you’ll find it useful.

If you do, you can support my work by buying my books and recommending them to others.

And by the way, there’s one thing we don’t say often enough to older women who get pregnant:

Congratulations 🥰💕😍🎉

👩‍⚕️ Is it better to see the same midwife or a small team of midwives throughout pregnancy and labour?

Evidence from 17 studies involving 18,533 randomized women was included in this updated systematic review.

Women or their babies who received midwife continuity of care models (seeing the same midwife or team of midwives) were:

⚕️ Less likely to experience a caesarean section or instrumental birth with forceps or a ventouse suction cup.

⚕️May be less likely to experience an episiotomy (a cut made by a healthcare professional into the perineum and vaginal wall).

⚕️More likely to experience spontaneous vaginal birth.

⚕️Reported more positive experiences during pregnancy, labour, and postpartum.

⚕️Additionally, there were cost savings in the antenatal (care during pregnancy) and intrapartum (care during labour and birth) period.

Further evidence may change our results, and future research should focus on the impact on women with social risk factors and those with medical complications, and understanding the implementation and scaling up of midwife continuity of care models, with emphasis on low‐ and middle‐income countries.

I regularly recommend humming and other vagus nerve exercises to my telehealth patients as gentle yet powerful tools for nervous system regulation, calming inflammation, hormone balance and supporting gut health.

Humming instantly spikes nitric oxide, tones the vagus nerve, lowers cortisol, releases stored tension, and activates rest-and-digest mode — making it incredibly effective for anxiety, insomnia, trauma healing, and chronic stress. I also recommend complementary practices like deep breathing, gargling, and singing, paired with vagal nerve stimulation devices, to improve heart rate variability, reduce inflammation, balance hormones, support gut healing, and ease autoimmune symptoms. These simple daily tools create the internal safety your body needs to move from survival mode into true repair and vibrant health.

It is so important to know the difference and a home birth and a free birth are different things…

Professor Dahlen says the research shows when women have low-risk pregnancies, planned homebirths attended by competent midwives are safe.

Professor Dahlen says increasing education about freebirthing isn't about impeding on a person's reproductive rights, but making sure they are "fully informed and given all the options available".

While freebirths and planned homebirths both happen outside of a medical setting, they are different birthing experiences.

Experts say creating awareness around how the two differ is important in helping women make informed and safe decisions.

I have been MIA on socials as life has just been well…busy! And it seems many I speak to are in the same boat.

Regardless I thought this deserved a spot on the grid to say I am now taking private midwifery clients on in 2026. Covering the western suburbs of Adelaide and it is in a limited capacity for now.
I am already grateful for all the enquiries and bookings I have had already.
I am almost fully booked for the first half of 2026, with only a couple of spots left for March.
I am not taking on any clients for February as my youngest is starting school. I am having time off at the end of July/beginning of August for The Convergence of Rebellious Midwives so I am unavailable during that time.
It is strongly recommended to contact a private midwifery as early as possible if you are considering Homebirth as many do book it early.
Please feel free to contact me to ask any questions and if you have an enquiry.
