This World Diabetes Day, we highlight Calvary’s integrated model of care which allows hospital patients, home care clients, and aged care residents, access to specialised diabetes education and support – wherever they may be.
Calvary Home Care Diabetes nurse Anne offers specialised education for patients, as well as support for families. For home care clients - including SA Health and NDIS - in the community, this support can be ongoing through their package. She also supports general clients who have slow-healing wounds, because diabetes is a major factor to poor wound healing, and Anne has expert knowledge in this area too. For Calvary Aged Care residents, Anne’s specialist expertise is available when residential facilities have difficulty accessing support for their residents.
Anne also provides support and education for hospital in-patients, where she may cover onsite staff leave, or attend when other specialised staff are not available.
“There was a patient here at Calvary North Adelaide Hospital recently who was from a remote area of the state. We were sent a referral from the hospital to see her while she was there, as post-discharge support was potentially quite difficult for this patient given where she lived. I was able to see her over several days and provide one-on-one education so that she felt much more confident. It’s about setting the person up to have all of the tools that they need to be able to manage themselves before they go home.”
Anne’s Clinical Team Leader Bec looks after the Home Care specialist nurses who cover stoma, diabetes, and wound care. Having the expertise of this group of nurses also available across Calvary’s services in hospital and aged care means patients, clients, and residents all have access to the highest level of specialist care.
