Meet Nicole Jones, Stomal Therapist and Specialist Wound Nurse at Wound Innovations, supporting patients and care teams across Adelaide.

Nicole began her career in wound care more than 13 years ago on the vascular ward at The Queen Elizabeth Hospital, where she was mentored by one of South Australia’s senior vascular nurse practitioners. This early experience built a strong clinical foundation and sparked her ongoing passion for complex wound management.

Following her postgraduate training, she expanded her scope of practice to include stoma siting, patient counselling, pre and post operative care, and advanced wound management.

Nicole has been part of the Wound Innovations team for the past two and a half years and greatly values both the practical and educational aspects of her role. She finds it especially rewarding when clinical knowledge truly clicks for nursing teams, those moments that build confidence, capability and stronger wound care practices.

Seeing wound healing progress and sharing in the excitement of positive outcomes with facility staff continues to drive Nicole each day. She takes great pride in knowing her expertise helps strengthen clinical practice and improve patient care across the community.

Wound Insight Tuesday 💡

Redness does not always mean infection.

Erythema may be caused by a range of factors including friction, excess moisture, sustained pressure or sensitivity to adhesive products.

Accurate wound assessment requires evaluation of the full clinical picture rather than relying on a single isolated sign. Consider wound history, exudate levels, temperature changes, pain, systemic symptoms and contributing environmental factors before determining appropriate management.

Taking a structured and evidence based approach supports timely and appropriate clinical decisions.

When Should You Refer?

Not every wound needs specialist care. But some absolutely do.

Here are 4 signs it may be time to refer:
• The wound has not improved in 2 weeks
• There are repeated breakdowns
• There are signs of infection
• Dressing plans are not achieving progress

Early intervention can prevent complications and hospitalisation.

If you are unsure whether a referral is appropriate, we are always happy to guide you. Message or send us an email to discuss a case or referral pathway.

Specialist wound care achieves the best outcomes when it is collaborative.

Complex wounds require more than isolated treatment. They require coordinated communication between clinicians, carers, support coordinators and families.

We work alongside:
• Aged care providers
• NDIS participants
• Support coordinators
• Community nurses
• Families and carers
• Clients

Clear communication and early intervention lead to stronger, more sustainable healing outcomes.

If you are supporting a client with a complex wound, our team would be pleased to work with you.

Message us to discuss collaborative care options.

Wound Insight Tuesday 💡

Prevention is just as important as treatment.

Skin integrity can deteriorate quickly without:
• Regular repositioning
• Appropriate pressure management
• Hydration
• Barrier protection
• Early risk assessment

Preventative strategies reduce escalation, hospitalisation and discomfort.

What preventative strategy do you prioritise most in your setting? Share below.

Understanding the 4 phases of wound healing is fundamental to effective clinical management.

Wound healing occurs in four overlapping phases:
• Hemostasis – stops bleeding
• Inflammation – cleans and protects
• Proliferation – builds new tissue
• Remodeling – strengthens scar tissue

Each phase plays a vital role. Disruption or delay in one stage can impact overall progress.

Recognising which phase a wound is in helps guide appropriate dressing selection, timing of intervention and referral decisions.

Save this post for future reference or share it with your team.

If you are unsure whether a wound is progressing appropriately, our team can help.

Wound healing is not always linear.

Some days look better. Some days look worse. Progress does not always move in a straight upward direction.

Fluctuation does not automatically mean failure. It often means the wound is responding to multiple internal and external factors.

Understanding this helps carers avoid unnecessary panic and focus on consistent, appropriate management.

Have you seen a wound fluctuate before improving? Save this post for future reference or share it with your team.
