✨ New Year Update – Now Taking Referrals ✨

As we head into the new year, I’m pleased to share that I will have capacity to provide Level 3: Specialist Support Coordination under the NDIS.

This service is available for plan-managed and self-managed participants who:
▪️Have complex support needs
▪️Are navigating multiple services or systems
▪️Need higher-level coordination to maintain stability and continuity of supports

Specialist Support Coordination focuses on:
▪️Reducing barriers to accessing and maintaining supports
▪️Coordinating multiple providers and services
▪️Supporting planning, risk management and crisis preparedness
▪️Strengthening choice, control and long-term independence

Referrals are welcome from participants, families, plan managers and allied health professionals.
