A 62-year-old woman with insulin-dependent diabetes for 55 years and biopsy-confirmed necrobiosis lipoidica diabeticorum (NLD); leg ulcers had been present for 2½ years.

After 37 HBOT sessions, the ulcers showed clear healing progress, with a more settled result at 3 months.

NLD is not a UHMS-recognised indication for HBOT; decisions are individual and HBOT is used as part of an overall wound and skin management plan.

One of the lesser-known, UHMS-recognised uses for HBOT is supporting compromised skin grafts and flaps; when tissue is struggling after surgery or trauma, HBOT can increase oxygen delivery to stressed tissue and support healing while the underlying issue is addressed.

It is not a replacement for good surgical care or blood flow assessment; it’s used as an adjunct when clinically appropriate.

If a continues to bleed, keeps soaking through dressings, or re-bleeds easily, it needs prompt medical review.

HBOT can be life-changing in the right setting, but it isn’t a universal answer. The evidence base is strong for certain recognised indications and still evolving for others; that’s why some clinicians are enthusiastic in the right cases, while others are cautious if the indication is outside established guidelines.

The most helpful question is often, “Is my condition a recognised indication; and if not, what evidence and goals are we using to guide care?”

A 47-year-old woman with diabetes and neuropathic symptoms, 10 days after stepping on a tack; the wound was extensive and at risk of deteriorating. Three weeks later, after 15 HBOT sessions, the wound bed shows healthier granulation and early healing progress.

HBOT may be considered as part of a broader diabetic foot wound plan when clinically appropriate; outcomes vary and careful wound care and offloading remain essential.

With , nerve changes can reduce pain, so a wound can worsen without much discomfort. If a “painless” blister, cut, or ulcer is enlarging, looking darker, getting wetter, or developing new odour or redness, it needs prompt assessment.

Do not rely on pain as your guide; visual changes matter more.

HBOT began with clear uses in diving and toxic exposures; but as mechanisms became better understood, formal indications expanded.

Today, UHMS-recognised uses include conditions such as decompression sickness, carbon monoxide poisoning, selected problem wounds (including some diabetic foot ulcers), compromised grafts/flaps, necrotising soft tissue infections, thermal burns, and delayed radiation injury; always as part of a wider treatment plan rather than a stand-alone fix.
