Dear outgoing residents, you have been sorely missed!
We have had some of you ask about ceasing RMOA payments as you leave us. As talked about in the AGM, we are attempting to make the cessation of payments smoother in the future, and also to assist people who have clearly left our hallways a few years ago. We are currently in talks with payroll and MWU in terms of contacting previous doctors and payroll to do this.
You are welcome to wait for this initiative to occur, hopefully within the next 2 weeks or so.
Otherwise, you can do the following!
1) Fill out the deduction form located here: https://drive.google.com/open?id=0B9ZrwruiHC98azNuN3k1MkdFOFE. In the top left corner box of the grid write 'cease' (Previously when you joined you would have written 'add'
2) Fax it to the Parramatta office number on the form.
3) Keep a copy for your records
4) If there are any issues, contact payroll immediately.
