Eastern Melbourne PHN is a Primary Health Network funded by the Australian Government to improve the care and support people receive from health services. We work in partnership to improve the way services are connected and to ensure they are easy to access when people need them most. We work closely with health professionals, consumers and carers to scope the gaps, identify emerging community needs and purchase services that address these needs. We invest in a range of initiatives to make a difference in our priority areas including chronic disease, mental health, alcohol and other drugs, digital health, Aboriginal and Torres Strait Islander health, immunisation and general practice support. We aim to improve the health of our community by ensuring people receive the right care, in the right place, at the right time. Our community of 1.6 million people live in Melbourne’s east and north east, from the inner-eastern suburbs of Melbourne, to the Yarra Valley in the east, and semi-rural communities as far north as Kinglake. www.emphn.org.au

We’re thrilled to share some exciting news.

EMPHN’s Multicultural & Homelessness Health Needs Assessments (HNAs) have received a commendation at the recent Planning Institute of Australia Awards for Planning Excellence in the Community Wellbeing and Diversity category!

This recognition highlights the work we are doing at EMPHN to better understand and address health needs of the community.

Our HNAs identify practical, community-driven solutions, ranging from outreach health services to culturally safe communication and navigation supports. Strong engagement with people with lived experience enabled us to capture the invaluable voices and expertise of a diverse group of community organisations and healthcare professionals across the EMPHN catchment, ensuring these insights truly reflect the needs of the people across the communities.

A big thank you to Larter Consulting and SGS Economics and Planning for their collaboration on this important project.

We recently launched our 2025 Allied Health Needs Survey and we would love to hear from you! This year’s survey aims to deepen our understanding of the allied health workforce across our region.

By participating, you’ll help us:
✔️ Identify the challenges and opportunities you face in your practice.
✔️ Strengthen multidisciplinary collaboration with general practice and local health networks.
✔️ Tailor our support, resources, and advocacy to better meet your needs.

New statewide referral criteria are now in effect for children’s surgery conditions in public hospitals. The Victorian Department of Health has developed and published statewide referral criteria for nine children’s surgery conditions.

The aim of statewide referral criteria is to improve access to public hospitals in Victoria by providing clear and concise guidance on when a referral for specialist opinion and management is indicated.

On Wednesday 19 November, Diabetes Australia will be delivering the Chinese Living Well program at Box Hill Library.

Chinese Living Well is a free diabetes education event for people living with type 2 diabetes. The event provides an opportunity to hear directly from health professionals, including dietitians and diabetes nurse educators, access in-language and culturally adapted resources and connect with peers.
