Research suggests a large proportion of people diagnosed with knee osteoarthritis haven't tried any structured exercise — even though it's one of the most effective treatments available.

It's not laziness. It's usually fear. Fear of making things worse, of doing it wrong, of being told nothing will help anyway.

Why 'rest' is often the worst thing for your knee osteoarthritis (OA).

The question last Friday: who's more likely to get knee OA - a runner or a desk worker?

The answer: The sedentary desk worker

Why?

The body is amazingly strong and adaptable. Unlike your muscles, your cartilage doesn't have its own blood supply. It relies on a "squeeze and release" mechanism - much like a sponge - to get nutrients and stay healthy.

- The runner: Is 'feeding' the joint through movement
- The desk worker: The joint is effectively starving for movement

If you’ve been told to "rest" your knee, you might just be letting the sponge dry out.

Sometimes it does.

Often there's quite a lot that shifts once someone understands what's actually driving the pain rather than just responding to it.

Most people think they’re 'wearing out' their knees by walking. It’s actually the exact opposite. Think of your cartilage as a living sponge. It doesn't have much of a blood supply—it relies on you to move to 'squeeze' nutrients in.

If you stop moving to 'save' the joint, you’re actually starving it.

"I've got knee osteoarthritis and my knees are deteriorating".

I hear this a lot.

And that word — deteriorating — is doing a lot of heavy lifting that it probably shouldn't.

It sounds like a one-way street. But knee arthritis is actually more dynamic than that. There are ups. There are downs. And there are things that genuinely influence how your knee feels and functions.

On Friday I asked: when your knee osteoarthritis flares up, what's your first instinct — rest, keep moving, pain relief, or worry?

All of those instincts make sense, and most people have tried all of them at some point.

Here's what tends to surprise people though: gentle movement usually beats complete rest, even during a flare-up.

Not pushing through. Not ignoring what the knee is telling you. Just keeping things gently moving so the joint doesn't stiffen up and the muscles around it stay switched on.

Poor sleep and knee osteoarthritis pain have a relationship most people don't know about.

When sleep is disrupted, the nervous system becomes less able to regulate pain signals. The same knee, on the same day, can genuinely be "playing up" more after a bad night than a good one — not because anything has changed in the joint, but because the system processing the pain is running on empty.

Improving sleep isn't just good general advice.

For knee osteoarthritis, it's part of the treatment.
