Progesterone is a true shape-shifter and the most enigmatic of hormones. It's responsive, relational, and endlessly fascinating. And it can behave entirely differently depending on estrogen, gut health, the state of the nervous system, and many other factors.

• In the uterus, it can either enrich the lining or keep it thin and quiet.

• In the brain, it can calm GABA receptors or, in some women, stir anxiety.

• In metabolism, it both boosts metabolic rate and temporarily reduces insulin sensitivity as part of the body’s adaptive rhythm.

It also increases the body’s requirement for sodium, protein, rest, and basically everything. So, when you're making (or taking) progesterone, it will demand MORE of everything you need.

Regular ovulation is how women make the hormones estradiol and progesterone, important for general health, not just to make a baby.

By suppressing ovulation, most hormonal birth control switches off those hormones and does NOT replace them.

Estradiol is our main ovarian estrogen and one of our most charismatic hormones. Here are a few key insights. 

🟡 Estradiol is highly beneficial for the brain, bones, muscles, metabolism, and more. It works best in partnership with progesterone. 

🟡 The women who benefit most from estradiol are young women (as in, young women benefit from the estradiol they make). Unfortunately, most methods of hormonal birth control SWITCH OFF estradiol and either don't replace it or replace it with ethinylestradiol, which does not have the same benefits. Young women being robbed of their own estradiol and progesterone is one of the women's health issues I'm most passionate or concerned about. 

🟡 Estradiol is such a strong mood stimulant that higher peaks (such as during the early phases of perimenopause) can be overstimulating. As I say in Period Repair Manual, "Estradiol is like an interesting and charismatic friend: great to have around but overwhelming after a while."

🟡 High estradiol can also trigger a histamine response, especially if there's not enough progesterone to balance it.

🟡 Even in young women, estradiol drops at the end of every menstrual cycle to very low levels, basically menopausal levels. Descent into this “estrogen trough” can cause estrogen WITHDRAWAL symptoms, such as anxiety, night sweats, and migraines, especially during the early phases of perimenopause when estradiol spikes up to 3x higher than normal. The higher estradiol goes, the further it has to fall, and estrogen therapy can be helpful during those week-long troughs. (The estrogen troughs of perimenopausal women are not as low as the troughs of young women, but the drop is steeper.)

🟡 In the later phases of perimenopause (when there are long gaps between periods), estradiol becomes more continuously low but still spikes up. In theory, the brain and body can adapt to that lower estrogen (after all, we evolved to have menopause). In practice (for reasons I describe in Hormone Repair Manual, especially insulin resistance), that adaptation does not always happen, and that's when estrogen therapy can be helpful.

All progestins slightly increase breast cancer risk, but experts had hoped that IUDs were safer because they release the progestin directly into the uterus and at a lower level than the pill. Now, a large study published in JAMA has detected a small risk from hormonal IUDs— "similar to that with oral contraceptive use," according to one expert.

Experts emphasize that the overall risk is still low: hormonal IUD use is associated with an “excess risk” of 14 cases per 10,000 women.

Do you make both ovarian hormones?

Estradiol and progesterone are not just for making a baby. They're also both important for general health.

And the only way to make progesterone is with regular natural *ovulatory* menstrual cycles. Contraceptive drugs are not progesterone and do not provide the same benefits.

Today’s acid-forming food = Tomorrow’s chronic disease?

All foods have an acidic or alkalising effect in the body, but it is crucial to know whether the diet is more acidic or alkaline so that a daily balance is maintained, as latent acidosis is linked to chronic disease and ill health.

The best way to improve acid-base balance in the body is via inclusion of more alkalising vegetables in the diet. Depending on the severity of acidosis and/or your stage of treatment, however, simply eating more vegetables may not be enough or may be a challenge!

You can compensate for the effects of excessive dietary acids using an alkalising mineral citrate formula. Citrates are ideal for alkalising the body in food and supplemental form. Citrates are always absorbed as a salt, with magnesium, potassium, calcium citrate, etc, and are found naturally in fruits and vegetables.

Supplementing with these citrate salts provides alkalinity with the added benefits of increased minerals. Long term alkalisation therapy with mineral citrates has been proven in clinical trials to benefit long term health across multiple body systems.

Fruit is a great source of nutrients and fibre. Try to choose low-sugar fruits, like berries or hard fruit that contain plenty of fibre (such as apples or pears - and make sure you eat the skin too, as that's where most of the health-promoting nutrients are found.

Fruit is also best eaten either with a meal or straight after, rather than as a snack, when it's more likely to spike sugars and be stored as fat.
