Thinking about changing your Support Coordinator?

Under the NDIS, participants have the flexibility to change providers at any time. But itโ€™s important to follow the right process to ensure your supports continue smoothly.

According to NDIS guidelines, this may include:
โœ”๏ธ Discussing the change with your current Support Coordinator
โœ”๏ธ Following the process outlined in your service agreement
โœ”๏ธ Ending your current service booking
โœ”๏ธ Creating a new service booking with your chosen provider

Your current Support Coordinator should also provide a handover report so your new provider understands your goals, supports, and plan details.
