To ensure the process is handled efficiently and, crucially, with complete clinical accuracy, we have a vital request regarding documentation completion.
We require all forms and certificates to be addressed and finalized during your scheduled in-person consultation or post-operative review appointment.
Why must paperwork be completed during the consultation?
This policy is necessary due to three primary, intertwined factors: strict patient privacy laws, the requirement for specialized clinical judgment, and the extreme time pressures of a modern medical practice.

Our administrative and reception staff are fundamental to the running of the clinic, but their access to your comprehensive Electronic Health Record (EHR) is severely restricted. This is a deliberate, legal measure to protect your private health information (PHI).
* Limited Access: Administrative personnel can only access necessary booking, billing, and contact details. They cannot access detailed clinical notes, laboratory results, surgical reports, or specific historical diagnoses.
* The Clinical Gap: Since forms require specific medical details (dates of surgery, exact diagnoses codes, specific functional limitations), staff without full record access are simply unable to fill them out completely or accurately, making the document invalid.
2. Specialized Medical Knowledge is Mandatory
Filling out certificates is not a clerical task; it is a clinical responsibility. Many forms legally require the signature of a treating doctor to certify that the medical statements made are true and based on direct assessment and expertise.
* Medical Interpretation: A doctor must interpret the requirements of the form (e.g., assessing fitness for a specific duty or determining the impact of a condition on mobility) against your current medical status. This requires current clinical knowledge and assessment that our administrative team does not possess.

* The Ripple Effect: If paperwork is dropped off and requires the doctor to squeeze in non-urgent administrative time, it inevitably creates delays in returning results, reviewing pre-operative fitness, or making phone calls for other patients. Completing it during your slot is the most time-respecting solution for everyone.
