When someone you love needs support at home, it shouldn’t feel like you’re managing everything on your own.

A coordinated care team means the people supporting your loved one are working together — sharing information, aligning goals, and adapting support as needs change.

Our latest blog explains why a team-based approach makes such a difference under Support at Home, and how it helps reduce stress for families and carers.

đź’™ Clear. Connected. Support that makes sense.

New year. New goals. Same knees. đź’š
And that’s okay.

Starting a new year doesn’t mean starting from scratch. For many people, progress is about building strength, confidence and comfort at a pace that suits their body and their life.

Under the Support at Home program, reablement and wellness support can help you work towards what matters most to you — whether that’s moving more comfortably, staying active, or maintaining independence at home.

If you or a loved one would like to learn more about support options, our team is here to help.

When it comes to home care, you might hear the terms reablement and maintenance — and wonder what they actually mean for you or your loved one.

Both play an important role under the Support at Home program, but they serve different purposes. One focuses on rebuilding confidence and ability, while the other provides reliable, ongoing support when needs are long-term.

Understanding the difference can help you make informed choices and feel more confident about your care.

A very special person and someone we are very sad to lose. It was an honour and a privilege to support this beautiful person and he will be dearly missed by the Braidwood community and by the Right at Home team ❤️

Vale Ken Thomas
St. Bede's Parish Braidwood reports:
It is with great sadness that we share the news that Ken Thomas passed away this morning in Braidwood, aged 84.
Ken was a much-loved parishioner, friend, and community member whose life was marked by service, generosity, and quiet faith. He devotedly cared for his first love Joan and Mother in law Phyllis at the end of their lives, as well as his beloved partner Jenny, whom he cared for with great love as she lives with dementia. His capacity to give of himself to those he loved was extraordinary.
Ken volunteered tirelessly across many organisations, including St Bede's Braidwood Parish, Braidwood Lions Club, Ryrie Park Markets, Braidwood Show Society, and numerous horse clubs and shows.
He will be fondly remembered for his caring nature, his willingness to help, and his affection for his friends.
Around a decade ago, Ken returned to his faith, which became deeply important to both he and Jenny and a source of strength and comfort in his later years.
Ken will be sorely missed by his family, friends, parish community, and the many groups he served so faithfully. We keep Jenny, his family and all who loved Ken in our prayers.
Eternal rest grant unto him, O Lord,
and let perpetual light shine upon him.
May he rest in peace.

Your Support at Home statement may look a little different — but its purpose remains the same.

Each month, your statement helps you understand:
✔ what services you’ve received
âś” how your funding is being used
âś” how much of your budget remains

To help make sense of it all, we’ve put together a simple guide explaining what your Support at Home statement shows and how to read it with confidence.

We’ve also created an easy checklist highlighting the top 6 things to check each month, so you can feel informed — not overwhelmed.
