Who are we? Aboriginal Health Worker have the ability to Advocate, Support, Refer on behalf of You or your family with consent and as you advise. We provide Cultural Safety, Awareness, & Respect in all your care needs.

What we do?AH respects confidentiality and practices this under SNSWLHD code of conduct. The role of the Aboriginal and Torres Strait Islander Health Worker is crucial to improving better health outcomes of Aboriginal and Torres Strait Islander people and minimize future presentations to hospital. AH can provide clinical, primary care, education and support for individuals, families and community groups when require. We want to make your stay in hospital as comfortable and convenient as possible and ensure when you are discharged you understand your care plan and have services to best support you on your return home. We can offer Hospital bookings, Home visits, Liaising GP and specialist appointments & care, Referrals to other services for example Centrelink, Housing, Dental, Eye care, Otitis Media (Hearing Screens), Diabetes Educator, Palative Care, Cancer Care, Community Health Nurses, and all services within the hospital. Or even to just have a yarn, we are always here for you.

When should I come? When you are admitted to hospital or walk in.
