ℹ️ CHN hosted a Commonwealth Psychosocial Support Program (CPSP) Forum for our commissioned providers, Directions Health Services and Wellways Australia. The Forum provided an opportunity for the CPSP teams to come together and discuss and dive into some of the challenges and complexity associated with their programs, as well as time to share stories, ideas and celebrate their successes. Our thanks is extended to Flinders University's CPSP Empower team for facilitating a very informative forum.

This week we welcomed visitors from NADA (The Network of Alcohol and Other Drugs Agencies) to Canberra. We didn't quite snap pictures at each of the Directions Health services that we visited, but it was great to catch up in person and show our NADA colleagues our ACT primary health, mental health, young people's and adult AOD treatment, support and harm min services.

Chat with PAT: where pathways and palliative care collide (in a good way!)

Clare Holland House recently welcomed Directions' PAT mobile clinic, which supports those in our community experiencing homelessness.

Directions' RN, Katie, was eager to explore pathways for clients who are older, homeless and experiencing life-limiting illness. Katie and PAT visited Clare Holland House to spend some time with the team and share challenges and discuss ways we can be better supporting clients experiencing homelessness - highlighting the power of networking across the jurisdiction to strengthen palliative and end of life care for vulnerable people.
