The most common cause of vertigo is Benign Paroxysmal Positional Vertigo (BPPV).

BPPV occurs when tiny crystals in the inner ear become dislodged and move into one of the semicircular canals. When you change your head position, these crystals shift, sending a false signal to your brain that you’re spinning.

This triggers an involuntary eye movement called nystagmus. The pattern of this eye movement tells us which canal is affected. The affected canal determines the most appropriate treatment approach.

At Ears in Balance, we use videonystagmography (VNG) to accurately record and analyse these eye movements, allowing for precise diagnosis and targeted treatment.

Vertigo is not a diagnosis.

Vertigo is a symptom, not a condition.

It describes the sensation of spinning or movement when there is none, but it can be caused by many different underlying issues.

Why this matters.

People often say “I have vertigo” when what they really have is:

BPPV (Benign Paroxysmal Positional Vertigo)
Vestibular migraine
Ménière’s disease
Vestibular neuritis
Vestibular labyrinthitis
Or even non-vestibular causes (neurological, medication-related, etc.)

Because of this, treating “vertigo” without identifying the cause often leads to incomplete or ineffective management.

Many of these conditions involve the inner ear.

Did you know your balance system plays a crucial role in keeping your vision steady during head movements?

This is thanks to the vestibulo-ocular reflex (VOR), one of the body’s most important balance reflexes—and one we can test using specialised goggles. If the VOR suddenly stops working on one side, it can cause a spinning sensation. If it stops working on both sides, your brain struggles to distinguish movement properly.

Why does this matter?
Because many vestibular disorders are invisible—symptoms like dizziness, disorientation, and imbalance often can’t be seen from the outside. By measuring the VOR, we can reveal what’s happening behind the scenes and make those hidden issues visible, so people can finally get answers and the right care.
