The widening between the base of the metatarsals consistent with a Lisfranc injury. This ligament is important for the stability of the midfoot and the arch. Gross instability can lead to long-term arthritis.

Is patient had significant hypermobility and despite a previous lateral ligament stabilization with suture brace they progressively loosened and became unstable again.

I had already demonstrated the LARS reconstruction technique but just wanted to show a better picture of the location of the tunnels used on x-ray. The lateral demonstrates position of the CFL tunnel and the pre-existing anchor in talus used for the suture brace, so had to go slightly more anterior for the new ATFL attachment. The AP x-ray shows the fibula tunnel.

We welcome Dr Mirun Thayaparan, our first of many Fellows in the Brisbane Foot and Ankle Fellowship.

Dr Thayaparan is a South Australian trained surgeon looking to sub-specialize in foot and ankle surgery and has just completed an arthroplasty Fellowship in hips/knees.

He will be with us for 6 months learning the dark arts of complex/MIS foot and ankle, he had a baptism of fire with a complex total ankle replacement on our 1st list together.

Achilles avulsion from the attachment to the calcaneus, rare but devastating injury. The principal remains that the tendon needs reattachment to bone, however this is not always that easy, and so as reinforcement, a LARS graft was utilized as an only patch to encourage healing to bone, while the tendon was repaired using anchors through a standard double row speed bridge type repair.

The rare case where an MIS bunion can progress to a non-union.

The initial metal work required removal and the non-union site was debrided back to healthy bone and multiple drill passes to stimulate new bone.

A allograft cancellous buying block was used to fill the defect and stop the first Ray from over shortening. The central hole was drilled and filled with auto graft from the heel.

New year, new ankle replacement. The x-rays were deceptive, showing only relatively mild valgus, but the plan showed otherwise, confirming a 13 valgus tilt.
MRI showed patient's tib post tendon was ruptured as well, so a varising calcaneal osteotomy and Cotton osteotomy performed with deltoid repair to alleviate the valgus.

Ankle fusions can give very good pain relief however adjacent joint arthritis can occur which led to us taking down this ankle fusion and converting to an ankle replacement.

Good range of motion at the end, hopefully the patient will be satisfied with the increased movement and indirect offloading of the adjacent joints.

Sometimes ankles are chronically unstable and with either a previous failed reconstruction or in particularly the ligamentous lax, an augmentation with LARS is required.
The initial tunnel is placed in the calcaneus with a stab incision, held with an interference screw.
Through the standard lateral incision the ligaments are taken down and repaired back with 2x anchors Brostrom style.
A tunnel is drilled through the fibula in an AP direction, after which the LARS is retrieved passing under the peroneals and through the fibula tunnel from PA. This is similarly held with an interference screw.
Finally the tunnel for the ATFL is found under II guidance and the LARS passed through lateral to medial, held with one last interference screw.

Trimalleolar fracture dislocation with syndesmosis involvement. Quite a large vertical shear fragment off the medial malleolus. Might have preferred a buttress plate medially but this was not available, so screws it is instead.
