Ptosis repair with frontalis slings

This lady had low eyelids that covered her pupils even when she tried to open her eyelids maximally.

At the time of assessment, it was noted that her levator muscle worked poorly so the standard eyelid lifting surgery would likely fail. Instead, a frontalis sling procedure was performed which connected her eyelid margin to her forehead muscle to allow her eyelids to lift and improve her vision.

Lower lid blepharoplasty in Thyroid Eye Disease

Typically lower eyelid blepharoplasty is considered a cosmetic procedure. However, in patients with thyroid eye disease it can be considered an important part of their surgical rehabilitation for their disease and is eligible for Medicare and health fund rebates.

This patient with thyroid eye disease has had a lower lid blepharoplasty following strabismus surgery.

BCC lower eyelid reconstruction (Wendall-Hughes flap)

This shows a lower eyelid basal cell carcinoma, which was then removed by a Mohs micrographic surgeon ensuring clear margins, with a defect involving the full width of the lower eyelid. The reconstruction performed is known as a Wendall-Hughes flap which is a two stage reconstruction. In the first stage the upper eyelid donates tarsal plate to the lower eyelid whilst remaining attached to the upper eyelid, and a skin graft is placed on the anterior surface of the defect. A second procedure a month later separates the eyelids - with both the upper and lower eyelid able to function normally. The end result you can see looks amazing.

Ectropion repair
This patient has previously had a skin cancer removed and reconstructed near her eye (with another surgeon). As she recovered, the lower eyelid margin pulled down and out. She had symptoms of a blurry and watery eye - as well has being concerned by its appearance.

Surgical correction of the lower eyelid position borrowed skin from her upper eyelid which you can barely notice.

Epiblepharon repair
**Look closely at the eyelashes on the lower eyelid**

In the before photos, the eyelashes are vertical and touching the cornea which can cause irritation. It’s a fairly common situation in Asian children that many will grow out of as they grow older and their face elongates. If not, then surgical repair corrects the eyelash position away from the eye.

This is subtlety different to lower eyelid entropion where the eyelid margin rolls inwards.

Upper lid lowering and blepharoplasty in thyroid eye disease

After monitoring to ensure her thyroid eye disease was stable and inactive, this lady had upper eyelid lowering to address her upper eyelid retraction, and at the same time had her excess upper lid skin reduced (blepharoplasty). She loves her new eyes that now look normal having fixed that startled appearance.

Excess fat or lid laxity can give rise to the appearance of bags. This can be a major concern for some patients. Surgical correction of this is considered a cosmetic procedure apart from a few medical indications. It can be technically challenging surgery so it’s important to seek out a surgeon with expertise in the procedure.

This sign is most often seen in patients with Thyroid Eye disease (Graves orbitopathy). It can be unilateral or bilateral.

It can also occur after ptosis surgery, such as congenital ptosis repair.

Symmetry
When assessing the periocular region one important factor that needs consideration is symmetry.

In this example, a brow lift was performed on both sides, and excess skin was removed just from the right upper eyelid.

As you can see the result is symmetric and natural.
