South West Individual Support is a disability support service providing assistance to participants and families who are eligible for support funding through the NDIS. At South West Individual Support we bring together a team of highly skilled professionals that pride themselves in delivering high quality care that caters to the needs of individuals and their families. South West Individual Support offers flexible hours, which allows families and individuals to reach out whenever they need. South West Individual Support can assist participants from the very beginning of their NDIS process, including assistance with access applications, through to building a care team that will meet the needs of each individual. At South West Individual Support we focus on delivering high quality care by working with each participant in an open and transparent way to ensure each the goals of each individual are achieved. South West Individual Support was founded in 2021 by Fiona Balcombe, who has over 30 years of experience as a Registered Nurse working within the disability sector. Fiona's qualifications include a Diploma of Business and Business Management, Certificate V in Training and Assessment, Internal Auditor, OHS Trainer and Emergency Response. Please note that our offices will be closed over the Christmas period from 5PM on Tuesday 23 December 2025 and will re-open at 9AM on Monday 5 January 2026. If your matter is urgent, please contact our on-call service on 03 5234 0500 and a staff member will respond to you as soon as possible.

If you require crisis support, please contact your local GP, hospital, or mental health crisis team. Alternatively, you can contact Lifeline on 131 114.

Hi All! Expressions of interest have closed for our Jan 2026 School Holiday Program. The activities have been finalised and letters and information will be sent out to parents by our office closure on Wednesday 24th of December, so look out in your emails.
Please note that there was a large amount of interest for the upcoming holidays, so your child might not have been allocated to every activity they applied for.

We had a wonderful day yesterday celebrating the start of the festive season. Thank you to all our wonderful families for coming to the SWIS Christmas party. Only 25 more days until Christmas!! We hope you all thoroughly enjoyed your day 🎄🎁

Some highlights from this years teens groups, adult groups and holiday programs. We have enjoyed a range of shows, dinner outings, and fun kids activities including VR games, bounce, outdoor obstacle courses and crafting days.

Our teens have been working on exploring the Melbourne area and learning how to utilise public transport and planning activities for themselves, and have enjoyed a range of activities from Holey moley, to ice skating and exploring docklands.

Our calendar is looking full for the rest of the year, with the upcoming school holiday program, monthly teen groups, and twice monthly adult groups.

We look forward to all the upcoming activities, and hope everyone enjoys them as much as we do!

New Staff Alert!!

So excited to let everyone know that we have a new Staff member joining our SWIS team.

Please welcome Amy, to our team of support coordinators. Amy will be based in Warrnambool and work in the Warrnambool and surrounding areas.

About Amy: Amy is an experienced community services worker with a strong background in out of home care and aged care. She has over six years of hands on experience supporting children and young people in foster care, residential care, and kinship care, using a trauma informed and person centred approach. Amy also has over five years of experience in aged care, where she worked as a case manager supporting older adults to remain safely at home, as a personal care worker in a residential aged care facility, and as a team leader managing community care workers to ensure quality service delivery.

She holds a Diploma of Community Services, Certificate 3 in Individual support, along with Certificates 4 in Mental Health and Alcohol & Other Drugs.

Amy is committed to helping people live their best lives though personalised & meaningful support.

This month our teens group headed to Geelong to test out the VR tech games.

Teens had a fantastic time!!

No teens group next month due to school holidays, but the group will head out again in Term 2 for horse riding and escape rooms.
