Position: Enrolled Nurses (ENs) & Registered Nurses (RNs)
Location: Various locations across Ballarat, Ararat, Maryborough, and remote work in NSW and QLD
Employment Type: Casual
Key Responsibilities:
- Conduct a detailed and comprehensive health history assessment for each client.
- Identify and discuss with clients the referral pathways that could enhance their well-being.
- Seek and coordinate GP management plans, mental health management plans, and chronic disease management plans to ensure clients receive all the services available to them under the public health system.
- Act as an interface between the health system and the client, ensuring seamless communication and access to necessary services.
- Maintain accurate clinical documentation and comply with regulatory requirements.
- Work collaboratively with multidisciplinary teams to support optimal patient outcomes.
Requirements:
- Current registration with AHPRA as an Enrolled Nurse or Registered Nurse.
- Demonstrated experience in aged care, disability care, or community nursing (preferred).
- Strong communication and interpersonal skills.
- A commitment to person-centered care and patient advocacy.
- Valid Police Check and Working with Children Check (or willingness to obtain).
- Current CPR and First Aid certification.
- COVID-19 vaccination compliance (as per government regulations).

Key Responsibilities:
Provide personal care, assistance with daily living activities, and social support.
Facilitate skills development and training.
Video-call based mentoring for young people living with disabilities.
Maintain accurate documentation and communication with the care team.
Requirements:
A full, valid driver’s license and access to a reliable vehicle.
Flexibility to travel as required.
Current First Aid and CPR certificates (or willingness to obtain).
Current police check (within three years).
Valid Working with Children's Check.
Experience in disability support or a related field (preferred but not essential).
Certificate III in Individual Support or similar (preferred but not essential).
