The story from Aged Care Placement Services to Elder Folks. 🧡

When we first began as Aged Care Placement Services in 2012, our focus was on helping families navigate residential aged care. Over time, that work grew — and so did the needs of the people we support.

Today, many of the individuals and families we work with aren’t just looking to move into residential aged care. They’re looking to live longer in their homes with care and guidance. Our new name reflects that broader role.

What hasn’t changed is our approach — taking the time to listen, understand, and support people with care, respect and clarity.

Here’s a positive story from our homelessness prevention and housing support program:

Norman* is a 76-year-old man from the Northern Rivers who lived without stable housing for more than 15 years, relying on friends and family for temporary accommodation. Over time, the ongoing uncertainty began to affect his health and wellbeing, and it became clear he needed a safe, stable home with the right support to maintain his independence.

Securing housing was complex. Norman wanted to remain close to family and community, had no recent rental history, and required access to ongoing aged care support. Elder Folks worked closely with housing providers, aged care professionals and community services to develop a tailored plan that reflected his needs and priorities.

We helped Norman secure an affordable villa in an over-55s village in Lismore, close to family and community. With My Aged Care services now in place, he has stable accommodation, consistent support and the freedom to live life on his terms. Today, Norman feels safe, connected, and proud to finally have a place to call home.

Here’s a story of success from our Occupational Therapy program:

Elizabeth is a 63-year-old woman living with severe osteoarthritis and preparing for a double knee reconstruction. Her mobility and balance had significantly declined, leading to several falls particularly in the bathroom, where the layout was no longer safe for her to manage independently.

Elder Folks arranged an occupational therapist home assessment to identify practical, evidence-based modifications to reduce fall risk and support Elizabeth’s safety before and after surgery. The assessment focused on the bathroom and rear stairs, where Elizabeth was most at risk.

Recommendations included removing the bath and extending the shower recess for safer access, installing grab rails to support transfers between the toilet and shower, and adding non-slip mats to improve stability. These changes have reduced the risk of falls and help Elizabeth remain independent at home with greater confidence, safety and dignity.
