🏥 Leaving hospital shouldn’t feel uncertain especially when aged care isn’t an immediate option.

Across Queensland, many patients who would normally transition into aged care are instead being discharged under NDIS due to hospital bed shortages. This can feel overwhelming for families who just want a safe, supported next step.

At MMT Care, we help aged-care patients eligible for NDIS funding leave hospital safely, with dignity and confidence.
