Complex Regional Pain Syndrome (CRPS)

• Cause: Believed to be caused by damage to or malfunction of the peripheral and central nervous systems, often starting after an injury like a fracture or burn.
• Pain description: A persistent, burning or throbbing pain that is out of proportion to the initial injury. It can be described as stabbing, shooting, or feeling like an electric shock.
• Other symptoms:
o Extreme sensitivity to touch or cold
o Swelling
o Changes in skin temperature, colour (blotchy, red, or blue), and texture (shiny or thin)
o Changes in hair and nail growth
o Joint stiffness and swelling
o Muscle spasms or weakness
• Spread: The pain may spread to other parts of the limb or even to other areas of the body.

Using the NOI principles of pain management for CRPS.

Explain Pain (NOI)
• What it is: An evidence-based approach to pain education developed by the NOI Group that helps people understand how pain works in the nervous system.
• How it helps CRPS: By explaining the neurobiology of pain, this approach aims to:
o Reframe pain perception: Change the belief that pain is a direct indicator of tissue damage.
o Reduce fear and anxiety: Help patients understand why their pain is so severe and persistent.
o Increase possibilities: Show patients that there are other options beyond constant pain, helping them engage in more activities.
o Improve outcomes: Give healthcare professionals a therapeutic tool to help patients with chronic pain, including those with CRPS, by creating a framework for education and treatment.

Rich River Physio's Friday Focus

Plantarfasciopathy (Plantar fasciitis)

Plantarfasciopathy (previously called plantar fasciitis) is one of the most common causes of heel pain. It happens when the thick band of tissue under your foot, the plantar fascia, becomes overloaded near or at where it attaches to your heel bone.

It’s not just about inflammation it’s about load and how your fascia responds to stress plus how it recovers from this stress.


Anyone can experience plantar fasciopathy, but more at risk are:

o Adults aged 40-60 years
o Runners and walkers especially after increasing mileage or training volume
o People with limited ankle dorsiflexion- seen after ankle sprains or if have weak foot muscles
o This limitation places more load on the plantar fascia
o Those spending long hours standing on hard unforgiving surfaces
o Individuals with high body weight or weak foot muscles

Symptoms include:

o Sharp heel pain with first steps in the morning
o Pain after rest or activity
o Tenderness under the heel along the foot, especially on the inner side
o Stiffness that eases with movement but worsens again

Management

o Strengthen calves and intrinsic foot muscles while managing load
o Reduce excessive walking and running, running and walking on the same cambers and hard surfaces.
o Restore balance and correct biomechanics to distribute load evenly across the foot.
o Massage, stretching and moving the plantar facia
o Taping, plantar fascia socks, mobilising ankles and calves

Physiotherapy and plantar fasciopathy management
Ultrasound, shock treatment, strapping, acupuncture, correcting biomechanics, appropriate stretching, load and exercise advice are all modalities that can potentially help. It’s complex and requires a multifaceted approach to management and treatment.

Menopause is more than hot flushes!

Menopause and musculoskeletal health
Up to 70% of peri menopausal and menopausal women report Joint Stiffness, Widespread Aches, Tendon pain, Back pain.

Sarcopenia—the age-related loss of skeletal muscle mass, strength and function—is accelerated by hormonal decline.

The sharp drop in oestrogen at menopause accelerates bone turnover, favouring resorption over formation, which leads to net bone loss. Women can lose up to 10% of their bone mass within the first five years of menopause, significantly increasing the risk of osteopenia, osteoporosis, and fragility fractures and evidence suggests that 50% of postmenopausal women may develop osteoporosis.

Declining oestrogen levels are associated with reduced collagen turnover and impaired tendon resilience, increasing susceptibility to tendinopathy and reduced healing capacity.

As hormone levels fall, some women experience increased joint stiffness, pain, and reduced range of motion.

Many women in menopause report new or worsening joint stiffness, tendon pain, and conditions like frozen shoulder and conditions like plantar fasciitis. Women are twice as likely to develop frozen shoulder than men and emerging research suggests that withdrawal of oestrogens during perimenopause may be an important risk factor.

A healthy athlete is one who:
1. Balances training (often in multiple sports) with recovery
Look at the total weekly schedule and ensure there is sufficient time to adapt and recover. At least one day of rest from all sport per week

2. Ensures sleep is a priority
8+ hours sleep per night reduces injury risk in adolescent players

3. Supports training with adequate fuelling
Fuel (with food) both before and after training and games, to support physical activity activity and repair muscle tissue

4. Has a healthy and regular menstrual cycle
Missing a period, even with hard exercise is cause for concern

5. Is proactive at managing injury and illness
Seeks timely Physiotherapy and /or GP support for injury and illness, especially if on going or recurrent

6. Supports their mental wellbeing
Allows mental downtime outside of sport by maintaining hobbies, relaxation and social connection

It's the balance (vertigo) apparatus in your Inner Ear- It’s complex. Believe it or not, the image is of an actual scale model!

Rich River Physio and Acupuncture clinic is the longest established Private Physio practice in the region. The practice has three Physio's, an Accredited Exercise Physiologist and a Massage Therapist who have post graduate qualifications to enable them to skillfully manage the wide range of treatment conditions that are seen at the practice. We also attend to the Physio needs of Cohuna hospital There is a wide range of products for sale at the practice including braces and supports, pillows, tapes, orthotics and heat packs. We accept all DVA, TAC, Workcover and EPC clients
