Our Annual Report 2024-2025 is now available!

This year’s Report highlights the progress we have made so far during the first full year of our 2025-2027 strategy, as we continue to work to provide high quality, sustainable, and innovative person-centred care to our clients and broader community.

It also outlines our commitment to connecting with our clients and consumers to listen, learn and understand how we can improve to ensure the best possible outcomes for everyone who visits healthAbility.

Thank you to all clients and consumers who have joined our Community Champions Advisory Group (CCAG) and taken part in our research projects this year. Your time, honesty, passion, and willingness to collaborate have been integral in helping us to implement our strategy and learn and improve together.

In addition to client and consumer research, this Report references our ongoing focus on collecting and measuring client outcome and impact data. Next year we will be releasing a summary of this data as an impact report highlighting the health and wellbeing outcomes for our clients.

Thank you to our Board, staff, volunteers, partners, clients, stakeholders and community members for your contributions to this Annual Report. We value your support as we look ahead to a year of growing and innovating our services for the benefit of everyone in our communities.

✨ Home Care Packages is now called Support at Home — same trusted care, just a new name! 💙

As of 1 November 2025, the Home Care Packages program has officially been replaced by the Support at Home program. Designed by the Australian Government, it helps older Australians live safely, independently, and comfortably at home.

This flexible program offers more in-home care options to ensure quality support for those who need it most. Our team of care partners can help develop a personalised plan that may include personal care, home modifications, or extra support services, whatever you need to stay independent at home.

With nearly 50 years as a trusted community health provider, healthAbility continues to help older Australians and their families stay connected and supported in their homes and communities. 💙

This Christmas, we’re proud to be a collection point for CareNet’s Reverse Advent Calendar, helping fill hampers for local families in need.

Here’s how to get involved:
1️⃣ Pick up a box from our healthAbility Eltham Reception
2️⃣ Fill it with 20 non-perishable items — things like cereal, pasta, rice, long-life milk, canned goods, biscuits or coffee.
3️⃣ Return your box to healthAbility Reception by 20 November 2025

Team up with your local school, sports club, workplace or family at home to help us fill these Christmas hampers for CareNet.

CareNet will collect and distribute the hampers throughout our local communities to families who need a little extra support this Christmas.

Our healthAbility Repair Cafe - Whitehorse is back tomorrow - Saturday, 8 November, 10am-2pm at Strathdon House and Orchard Precinct, 449/465 Springvale Rd, Forest Hill.

Our team of amazing volunteer repairers will be available to help fix any of your broken items and restore them to their former glory. We hope to see you there!

Welcome to our new refurbished Eltham reception, including a fun kids’ space and Community Cupboard. A grant was sourced through the Victorian Health Building Authority under the 2023-2024 Metropolitan Health Infrastructure Fund. It brings this space in line with modern accessibility standards, including a centralised automatic entryway, inclusive and ambulant restrooms, an internal corridor linking our building and modified doors for easy access to our Womin Jeka activity space.

We're continuing the call for more investment in community health. With better facilities, we can make it easier for everyone – especially the most vulnerable – to get the care they need, when they need it.

Community Health is reaching more Victorians than ever before.

Our People, Place & Partnerships Impact Report showcases the collective achievements of Victoria’s registered, independent community health organisations and the growing demand for trusted, local care.

In the past year, over 674,000 Victorians accessed community health services — an 11% increase.

Community health is now the first point of contact for many Victorians, delivering care that is:
✅ Affordable

✅ Local

✅ Personalised

Community health programs are reducing emergency department visits, improving chronic disease management, and saving the health system money — with some programs returning up to $13 for every $1 invested.

This year’s theme, “You are, know, or will be one,” highlights how caring touches almost everyone’s life. We honour and celebrate the 3 million Australians who provide care to family members or friends.

Later in the month, we will be hosting free morning tea events with healthAbility’s Carer Support Program team, a wonderful chance to connect with other carers, share stories, and enjoy some well-deserved time for yourself. Plus, there’s a chance to win a door prize to support your wellbeing!

Established in 1976, healthAbility (previously Nillumbik Health) is an independent, community owned organisation. Everyone is welcome to visit healthAbility and use our services. healthAbility provides health, education and social support services for children, youth and adults in the North Eastern Area of Melbourne.Centralised at one location in Eltham, we exist to improve the health and wellbeing of people across all ages and abilities.
