Our senior Physio, Tom, recently completed training in the GLA:D Back program, designed for people with long-term or recurrent low back pain. The program combines education & exercise to improve spinal function, reduce pain & build confidence in movement. Its goal is to equip patients with the knowledge & tools to better manage their pain.
The foundation of this program is several key principles of managing & understanding low back pain. Key principle number 5 is the brain can turn pain up or down. Pain is influenced by two main factors: signals from the body & how the brain interprets those signals.
Turning pain UP
🔹The brain can release substances that can worsen the pain experience.
🔹For example, if you feel anxious, scared or depressed, the body creates chemical substances, such as the stress hormone adrenaline, which can expose the brain to more pain signals and at the same time inhibit the body from forming substances for pain relief.
🔹The brain can amplify pain information about a possible threat, sometimes when it is not needed. The signals from the back that the brain used to downplay are now interpreted as danger signals. You experience increased physical pain.
Turning pain DOWN:
🔹The brain can produce chemicals that are as pain-relieving as morphine (e.g. dopamine, serotonin & oxytocin).
🔹These chemicals act as pain-relievers when they are released into the bloodstream.
🔹In emergencies, the brain can reduce pain if it considers that something else is more important. E.g. in 2003 Bethany Hamilton was surfing & lost an arm due to a shark attack. She said later that she felt no pain. Why? The brain can reduce pain if it considers that something else is more important, like swimming to shore for survival!
🔹Your thoughts, feelings, experiences, beliefs, expectations & your mood play a central role in what chemicals are released by your brain & thus influencing the pain experience.
