🚐 Stories from the Van - Meet Lizzie 💙

Lizzie is one of our amazing Outreach Nurses who makes every visit count.

"I absolutely love meeting people and having a chat, helping them with small things like wound care to helping them access mental health services. I love our team and how dynamic and ever changing our role is. We genuinely care and have a really wonderful connection with our clients."

Every week, our van hits the road to offer care, support, and a friendly face to people doing it tough. It’s the small moments - a conversation, a check-in, a little help, that make a big difference.

Remembrance Day 2025

Today, we pause to honour and remember the courage, sacrifice, and service of all those who have served, and continue to serve, our country.

At Coast & Country Primary Care, we acknowledge the deep impact of their dedication on our communities and the freedoms we cherish today.

At 11am, we will join Australians across the nation in a minute of silence to reflect on their bravery and to express our gratitude.

🛠️ Physical Support Provided:
43 supports, 33 food/water, 6 clothing, 4 hygiene
