**** Patello Femoral Pain ****

One in 14 school-aged adolescents has patellofemoral pain, making it the most common non-traumatic knee pain for this age group. It is more common in females and those who are active, but can occur with those who have a sedentary lifestyle.

The key is the spike in activity above 'normal', which results in an overload of the knee supporting tissues and pain, most commonly the knee cap (patella). Research shows that if the cause of the pain isn't addressed, adolessents often cease participation in sport and activities.

By performing a compreshensive assessment, your Physio can identify the specific contributing factors and provide a customised rehabilitation program to prevent reoccurance. Short term techniques like graded taping, orthotics and dry needling can also be provided.

Did you know we offer dry needling for stubborn muscle pain?

All of our physios are trained in dry needling, and it’s something we use regularly to help clients who just can’t shake their pain. In fact, Dr Doug has been teaching dry needling to thousands of health professionals across Australia (and overseas!) for more than 25 years, and he still runs courses every year.

What surprises many people is that muscles can actually “refer” pain to other areas of the body. A heart attack is the classic example — the heart is a muscle, and the pain is often felt in the arm, chest, or neck. Women's Health Series:
** Stress Urinary Incontinence **

Do you leak when you sneeze or cough?
What about when you jump or run?

Musculoskeletal Pain Management & Medications

We know it can take 10-15 years for information to move from research into clinical practice, and part of this knowledge translation is achieved through educational posts like this.

While the following is not new research, I am surprised by how many clients present to the clinic saying they are using regular medications eg paracetamol, to manage their long-standing pain.

I appreciate that everyone is different, but it is essential to understand the risk profile of an intervention, e.g., medications, and also the alternative strategies and their benefits.

Firstly, looking at the benefits of a medication. The Number (of patients) Needed to Treat (NNT) is what is used to explain how likely a medication is to “work”. This compares how the medication reduces pain by half (50%) compared to a placebo (e.g. a sugar tablet or ‘dummy’). An NNT of 2 means that 1 patient in 2 treated with the pain-reliever will get a reduction in pain (by at least 50%), compared to a placebo. In the case of paracetamol, this is 4. This means that 1 in 4 people can expect to achieve at least a 50% reduction in pain. The other 3 will receive no benefit.

Medications also have side effects. The Number Needed to Harm (NNH) is what is used to explain how likely a medication is to give these side effects. An NNH of 8 means that 1 in every 8 people taking the drug will experience a side effect, compared to a placebo, because of the medication. For paracetamol, this is 12, so 1 in 12 people using paracetamol will experience an impact significant enough to warrant medical intervention.

The Research

This systematic review brings together all current research on the topic of using paracetamol for back pain and OA. The researchers pool the data from each study and analyse trends. It is considered the most potent form of research and helps to guide clinical decisions. The conclusion was straightforward, but it is a significant departure from past recommendations, which means it will take a while for the information to be accepted: "Paracetamol is ineffective in the treatment of low back pain and provides minimal short-term benefit for people with osteoarthritis."

This second paper is also a systematic review and the topic examined relates to osteoarthritis management of pain in lower limb joints - predominantly the hip and knee. The conclusion supports earlier findings. "As of 2002 sufficient evidence had accumulated to show a significant benefit of exercise over no exercise in patients with osteoarthritis, and further trials are unlikely to overturn this result. An approach combining exercises to increase strength, flexibility, and aerobic capacity is likely to be most effective in the management of lower limb osteoarthritis".
An important nuance here is the type and composition of the exercise undertaken, so that it is tailored to the individual person's baseline ability and pain plus practical things like equipment available.

We are celebrating our profession's birthday!

September 8 is World Physiotherapy Day, and we are focusing the month on reducing falls and improving strength capacity in older adults.

It is natural for our strength to decline as we age, in fact, we lose 10% of muscle mass per decade after 30. This rate can increase for women through menopause. While this is a natural process, like grey hair and wrinkles, we can influence the RATE at which this process occurs through strength training.

By staying strong, we reduce our fall risk, can garden for longer, hit the golf ball further, and pick up our grandchildren more easily. Life is better when we are strong!
