Operation: Delayed right sided breast reconstruction with deep inferior epigastric perforator (DIEP) flap.

Outcome: Before and after 5 week post operative. This patient previously underwent a mastectomy for right sided breast cancer. Her reconstruction was performed using her own abdominal tissue as a DIEP flap. Excess abdominal tissue was harvested on blood vessels, the flap was transfer to the chest where microsurgery was performed to re-establish blood flow, then the tissue was shaped into a breast. The abdomen was then closed in an abdominoplasty style to achieve a flatter contour.

Surgery Duration: 5 hours.
Hospital Admission: 1 week stay.
Recovery: Supportive bra at all time for 6 weeks.

Operation: Excision of lesion left medial leg and keystone flap. Before and after 4.5 months post operative.

Outcome: This patient presented with an aggressive squamous cell carcinoma (SCC) on the left medial leg. Because the lower leg generally lacks the laxity required for direct closure, I excised the lesion and reconstructed the area using a keystone flap. While a skin graft is an alternative, I prefer flap-based reconstruction in many cases as it tends to offer more robust healing. I frequently teach this flap technique at skin cancer workshops, as its staged design allows progressive movement and reliable closure.

Surgery Duration: 30 minutes
Hospital Stay: Day surgery
Anaesthetic: Twilight sedation + local anaesthetic

Procedure: Gynaecomastia excision and liposuction.

Outcome: Before and after 6 weeks post-operative. This patient had excess glandular and fat tissue to his chest. I removed the glandular component through a semicircular incision around his ni**le taking 45g from the left and 50g from the right. I then performed liposuction over his chest to address the fat component. The result is a flatter chest which is more proportionate to the rest of his body.

Procedure: Horizontal thigh reduction + VASER liposuction

Outcome: Before and after photos at 12 weeks post-operative. This patient requested a thigh reduction but did not want a scar down her thigh. I performed a horizontal thigh reduction, placing her scar in the groin crease, which can be concealed when wearing underwear/swimwear. I resected her medial thigh tissue, taking 120g from each side. I then performed VASER liposuction removing 250cc each side. I suspended her thigh using strong sutures onto her pelvic bone to achieve a lifting effect and prevent labial splaying.

Surgery Duration: 2 hours
Hospital Admission: Overnight stay
Recovery: Compression shorts at all time for 6 weeks.
