Last night we hosted a special session on navigating the new My Aged Care system, bringing together GPs, nurses and practice teams for an honest and practical conversation.

At the heart of it all was one shared goal: making sure people and families feel supported, informed and able to stay at home for longer with the care they need.

We unpacked what the Support at Home changes actually mean in real life, not just on paper, and how we can make the system easier to navigate for everyone involved.

What stood out most? That great care doesn’t happen in isolation. It takes collaboration, communication and people who genuinely care.

A big thank you to everyone who joined us and shared their insights.

Meet Shelley, our Community Engagement Manager.

From connecting with local GP clinics to bringing the team together beyond the workplace, Shelley plays a vital role in building the relationships that sit behind great care.

Her days range from coordinating presentations with healthcare professionals to making sure the team feels supported, valued and connected, because strong relationships don’t just happen in clinical settings.

As Shelley puts it, when you’re surrounded by people who genuinely care, that energy flows through everything you do.

As autumn begins, it’s a good time to prepare ahead of flu season, especially when supporting older adults.

Changes in weather, increased time indoors and seasonal viruses can place additional strain on ageing immune systems. Taking simple preventative steps now, such as booking a flu vaccination, staying hydrated and recognising early symptoms, can help reduce the risk of complications later in the season.

Preparation today helps protect well-being in the months ahead.

Use this guide as a helpful resource for families and carers.

Meet Abby, our Clinical Coordinator at Sovereign Community Nursing.

Behind every home visit, care plan and referral is someone ensuring everything runs smoothly: from clinical coordination and scheduling to answering enquiries and supporting the wider team (especially when it comes to food).

Abby’s role may span “a million different things”, but at its heart is one clear focus: making sure both our clients and our team are cared for and able to deliver the highest standard of care.

What makes it most rewarding? Working alongside people who genuinely care and share the same passion for making a difference.

Professional carers play an essential role in the lives of older adults, and their impact goes far beyond practical assistance.

From supporting daily routines and mobility to noticing subtle health changes and offering companionship, carers provide stability and reassurance during times of change.

Consistent, compassionate support helps older adults remain safe at home for longer, while also easing the pressure on families navigating complex care decisions.
Behind every care visit is more than a checklist; it is a relationship built on trust, presence and dignity.

Valentine’s Day is often about romance, but in later life, love can look like something quieter: checking in, noticing changes, and planning ahead with care.

For many older adults, this time of year can bring both connection and loneliness. Small shifts in mood, energy or behaviour can be easy to overlook, yet they may signal a need for additional support.

In aged care, professional love is shown through presence, continuity and expertise, and it can make all the difference to wellbeing at home.

Save this post as a gentle reminder that care is one of the most meaningful expressions of love.

Hot weather can affect older adults faster than we realise.

Signs like dizziness, weakness, reduced urine output, nausea or confusion may be early indicators of heat-related illness, and should never be ignored.

Knowing what to look for helps families and carers act early and keep loved ones safe.

A balanced diet plays a vital role in maintaining strength, energy and independence as we age. Regular meals with enough protein support muscle health, while fibre-rich foods aid digestion and heart health. Staying hydrated is just as important, as thirst can be harder to notice over time.

Small, consistent choices, like prioritising nutrient-dense foods and maintaining regular meal routines, can make a meaningful difference to overall wellbeing and recovery.

Share this post as a simple reminder that good nutrition is a key part of healthy ageing.

When school returns, family routines often shift quickly. Earlier mornings, busier days, and changed schedules can unintentionally impact the care and well-being of older relatives.

Missed check-ins, delayed meals, medication timing changes or fewer social moments can add up, especially for older adults who rely on consistency and routine.

Being aware of these changes allows families to plan ahead, keep care consistent and ensure older loved ones continue to feel supported and connected during busy transition periods.

Did you notice changes in a loved one over the holidays?

Extended time together often brings subtle shifts to the surface: changes in memory, mood or daily routines that may have gone unnoticed before.

These moments can be difficult, but they are also important. Early recognition, simple documentation and timely assessment can help families move from reacting to planning.

Music can do more than lift the mood; it can support memory, reduce stress and create meaningful moments for older adults.

Music therapy is increasingly used in aged care to help with:
• Mood regulation
• Memory recall in dementia
• Stress reduction
• Gentle movement and engagement

Whether guided by a trained therapist or woven into everyday routines, music can spark connection when words become harder to find.

Share this post for simple ways to bring the benefits of music into daily care.

Staying well at home starts with the right check-ins at the right time.

Regular health appointments can help older adults manage chronic conditions, reduce risks like falls or medication errors, and spot changes early, before they become emergencies.

From annual GP reviews and medication checks, to eye, hearing and mobility assessments, these appointments play a key role in supporting independence and wellbeing.
