This is the official page for South Eastern Melbourne PHN (SEMPHN). SEMPHN works on behalf of the Australian Government to improve health care for the benefit of 1.6 million people living in our region. We want everyone in our community live their healthiest lives. We support this by providing:

- Evidence, planning and influencing services to meet population health needs
- Capacity building services to grow primary health and its workforce in our region
- Commissioning services to translate national and state policy into local services and the regional health
system. Consumers, clinicians and other providers are integral to our efforts to improve local health care. Follow and engage with us to help make health care better.

Great to welcome Senator Lisa Darmanin and Emma Vulin MP to the Pakenham Medicare Urgent Care Clinic to mark its opening 💚

Since opening in December, the clinic has already supported almost 1,500 presentations — providing walk-in, bulk billed care for urgent, non-life-threatening conditions.

The Victorian Department of Health has developed and published statewide referral criteria for lung nodule condition.

The aim of statewide referral criteria is to improve access to public hospitals in Victoria by providing clear and concise guidance on when a referral for specialist opinion and management is indicated.

A new option for urgent care is now open in Carnegie 💚

The Carnegie Medicare Urgent Care Clinic offers free, walk-in treatment for urgent but non-life-threatening injuries and illnesses — and is already helping local patients get seen sooner.

It’s part of four new clinics across south east Melbourne, bringing the total commissioned by SEMPHN to eight Medicare Urgent Care Clinics.

In addition to producing a 3-year comprehensive health needs assessment, we undertake a voluntary annual update to ensure we, and our stakeholders, have access to the latest information and insights.
