When cardiology patients leave hospital, they’re often overwhelmed by new medications and side effects, and can’t get in to see their primary health practitioner❤️

As a result, 1 in 6 patients are re admitted, with half of these readmissions due to medication issues💊

The RECARD program was created to address this challenge. The trial brings together a multidisciplinary team of pharmacists, Indigenous health workers and nurse practitioners across Princess Alexandra Hospital, Royal Brisbane and Women’s Hospital, and Townsville University Hospital.

For 30 days after discharge, patients receive individualised care, including medication education, Home Medicines Reviews, GP and community pharmacy liaison, phone reviews and home visits.

RECARD is a powerful example of collaboration between The University of Queensland researchers and Queensland Health clinicians, bridging public and private care to improve outcomes for patients.

At the Southern Moreton Bay Islands Community Partnership Service, the team is proving that everyday actions can drive meaningful change this Earth Day. 🌍✨

During their daily client home visits, which are often made on foot, staff began collecting discarded cans and bottles along their routes. What started as a simple idea has become a daily sustainability habit. 🌱

“Over the past year, we’ve collected more than 2,000 bottles and cans," Nurse Unit Manager Rachael James shared.

The initiative is strengthening team culture, supporting the local community, and helping keep the islands clean. Funds raised from recycling now go toward shared lunches, building team connections while supporting local business.

“This initiative brings us together as a team and supports our community, while also helping keep the Southern Moreton Bay Islands clean as we do our part for the environment.” 💚

Meet Adiola Mazambani, Team Leader in Child and Youth Mental Health. ⭐

Recognised for her leadership, Adiola brings a unique and caring approach to mental health, shaped by her experience across the UK, Australia and Zimbabwe.

In her role, Adiola supports young people and families through difficult times, always focusing on connection and understanding.

Her work is guided by the idea of “Ubuntu” which means “I am because we are.” It reflects her strong belief that care is about people, relationships and community.

Adiola says every family she works with has helped shape the way she cares. She believes it is not just about clinical skills, but about listening, being curious and understanding each person’s story.

She also values teamwork and believes good leadership means growing together, supporting each other and creating a positive culture. 🩷

Meet PA Hospital Occupational Therapy Assistant Simonne. 👋

She's worked in the GARU wards for four years and has been recognised with a Staff Award for Compassion, celebrating her ability to bring joy to patients during their rehabilitation journey.

“I really enjoy supporting people,” Simonne said. “This line of work allows me to get to know people on a deeper level. I’m able to use my energy to uplift and inspire people in a creative way to achieve their leisure goals.”

With a background in music, Simonne brings a unique and personal touch to her role, whether that’s supporting a patient to keep playing their ukulele or leading group singing sessions on the ward.

“It’s about helping people reconnect with the things they enjoy,” she said. “That might be something creative, social, or simply getting out for a coffee if they need support with mobility.”

Congratulations Simonne! ⭐

In a demonstration of compassion in action, Acting Food Services Manager Letitia Green has coordinated a sweet surprise for patients who find themselves at QEII Hospital on their birthday. 🎂

"It came to my attention that patients here in the hospital were only getting happy birthday messages on their menus," Letitia said.

"Birthdays are special occasions that only come round once a year, so I thought we could do better. I decided to introduce a new process in Food Services so that each patient gets a red velvet cupcake on their birthday to celebrate as well."

QEII Culture Club Chair Sarah Stormborn said the initiative is an example of living our Metro South Health values.

"Letitia has done a wonderful job of partnering with consumers to make their stay with us just that much nicer," Sarah said.

"Kindness doesn't cost us much but it has a big impact on the people around us. If we can all think about one compassionate action we can take each day to improve the circumstances of our colleagues and patients, the benefits will ripple outwards."

Great work Letitia! 🫶

Logan Hospital’s Rehab team recently worked with patient Mara Paul and her family to host a private Australian citizenship ceremony on the ward.

Surrounded by family, friends and staff supporting her recovery, Mara officially became an Australian citizen after nearly 20 years in Australia.

“It was such a privilege to have my own ceremony here,” Mara said.

The ceremony ended with a special moment, as Mara’s husband John led a Māori hymn.

A lovely example of ways our staff go above and beyond to provide culturally safe, patient centred care. 💛

Advanced Sleep Scientist at PA Hospital’s Sleep Disorders Centre, Kevin Hanssen has had a unique insight into the mechanics of sleeping while on the job for the past 30 years. 😴

His interest in sleep medicine began during university as part of a clinical placement through his Masters of Clinical Physiology when it stood out as a fascinating and meaningful area within allied health.

Motivated by a desire to improve people’s health fundamentally through sleep, Kevin said it has been rewarding to see sleep gain greater recognition in the wider community as a vital component of overall wellbeing. 🛌

“There have been many highlights over the years including the privilege of contributing to sleep research, presenting at international conferences, and participating in clinical trials for new sleep medications,” Kevin said.

“But what stands out most is the people. Working alongside such a dedicated and supportive team, scientific, administrative, operational, and medical staff across both day and night shifts, makes every day rewarding.

Congratulations on 30 years making a difference Kevin! ⭐

PA Hospital Nurse Practitioner, Chunyan (Jen) Zhen is kicking goals in her specialty of Diabetes and Endocrinology and has now expanded her practice into Osteoporosis. 🩷

“Being a Nurse Practitioner is very rewarding,” Jen said 12 months into her qualification as an NP.

“Within my scope as an NP, I get to review patients with more complexity and have more clinical communication with other clinicians, particularly the specialists in haematology/oncology as well as immunology and gastroenterology. This makes the role more diverse.”

Jen says the sheer size of the hospital and the many patients who require complex case management across different specialties make working at PA Hospital enjoyable with adventurous approaches to diabetes care.

“I get to see different patients every day; this dynamic is quite rewarding at this stage of my career.”

Thank you, Jen, for the wonderful care you provide to patients! 👏

With more than 31% of our community born overseas and over 200 languages spoken, Metro South is proudly one of Queensland’s most culturally diverse regions. 🌏✨

Our future workforce is stepping up to meet that diversity with heart.

This year, Social Work students on placement joined the 'We speak your language' badge initiative for the first time, thanks to a Quality Activity Project led by our Social Work Clinical Educators and the Health Equity and Access Team.

“To date, three social work students across MSH have taken up a language identification badge, with languages spoken including Malayalam, Hindi, Punjabi and Vietnamese," shared Clinical Educator Sandra Young.

Students are embracing the chance to honour their culture, build confidence, and support CALD consumers, strengthening culturally responsive care across MSH today and into the future. 🌻

After 40 years in nursing and midwifery, Beaudesert Hospital’s Marg Wendt says it’s the people who have shaped her career. 💛

“I’ve been so lucky to walk alongside women and families during some of the most important moments in their lives, not just patients, but colleagues too,” she said.

From supporting new mums as a lactation consultant to leading services as Nursing and Midwifery Director, Marg’s impact has been felt across generations of care.

A proud local, Marg says Beaudesert Hospital is part of her community.

Congratulations on an incredible 40 years of service!

QEII Hospital’s soon-to-be-launched room service on demand project is set to deliver a key benefit - reducing food waste. 🍽️

QEII Food Service Dietitian Anita Mohanlal and Project Lead Karly Miller say the initiative aligns strongly with the MSH Environmental Sustainability Plan and global efforts to improve food security and environmental health.

According to the 2021 National Food Waste Strategy Feasibility Study, Australia wastes around 7.6 million tonnes of food each year, equating to more than 300 kilograms per person.

“At QEII, we know a large portion of food waste is driven by traditional hospital food models. Moving to room service on demand, QEII fundamentally changes that,” Anita says.

The new model will enable patients to order meals using their phone, the CBORD Patient App or a dedicated onsite contact centre. Meals will be prepared and assembled to order, rather than produced in bulk and held in advance.

“This shift means we are no longer producing meals that may not be eaten,” Karly says.

With the move predicted to reduce food waste by up to 50 per cent, that's great news for hungry QEII patients and the planet! 🌍
