A recent study showed that men and women were likely to stop using minoxidil within 30 days because of dermatitis-like reactions and extreme itchy scalp (Shardi, 2023).

Here’s what most people miss:

😌 Minoxidil doesn’t touch your hormones.

Zero. Zilch.

It makes the follicle size bigger so that vellus hairs can grow thicker in subsequent hair cycles. It doesn’t have any effect on DHT.

😵‍💫 Starting topical minoxidil is a trap.

This is a false consensus. Here’s the thing about topical minoxidil.

👉🏼 It’s like brushing your teeth. When you stop brushing, cavity and decay comes back. Same with hair loss.

This is how to improve your topical minoxidil (no holding back):

👉🏼 Leave on your scalp for a minimum of 10-12hrs. This means you need to apply twice daily. If you want to use it once a day, you need to apply 2ml x 5% once daily.

👉🏼 Combine microneedling once a week can increase Minoxidil effectiveness by up to 40%!!

👉🏼 Add Tretinoin improves absorption and also stimulates stem cells (except Minoxidil Plus+ because we added retinal).

👉🏼 Add oral saw palmetto or pumpkin seed to target DHT (except Minoxidil Plus+ because we already did that for you too).

🫨 Will PP405 revive dormant hair follicles in balding scalp?

Hey there, my name is Helen and I am a compounding pharmacist. Today I am going to breakdown what’s new in the hair loss space from a mechanistic perspective.

What I am excited about:

👉🏼 modified release minoxidil tablet: I am surprised drug companies haven’t done it already, but instead fought over the generics market.

👉🏼 alcohol-free and minoxidil-free minoxidil: because I am biased; just kidding. I believe it has a reason to exist.

What I am 50/50 about

👉🏼 PP405 - the mechanism is too close to Vitamin D and I wouldn’t be surprised if the results end up similar to increasing vitamin D in those who are deficient.

Minoxidil itching is caused by:

👉🏼 alcohol sensitivity
👉🏼 propylene glycol allergy or
👉🏼 all the above for some people

Ordinary minoxidil solution depend on alcohol and propylene to dissolve and carry minoxidil across the skin barrier.

Minoxidil foam has too much alcohol to compensate for no propylene glycol so it’s not that much better.

Switching to PG-Free and alcohol free formulation can resolve minoxidil itching and scalp irritation in as little as two weeks.

No corticosteroids.
No allergy tablets.
No messy moisturising scalp lotion.

How? Because you just removed the root cause.

Should Minoxidil 5% be applied once or twice daily?

Minoxidil half life in the skin is about 22hrs so theoretically, once daily dosing should not affect its efficacy - however, you should choose the regimen that you are likely going to be consistent with.

🔑 If you choose to do once daily dosing, we recommend that you use 2ml at night.

A study in 1987 found that patients who switched from twice daily to once daily after two years of using minoxidil experienced hair fall and had less hair count within nine months. The patients who switched to once daily dosing also halved their minoxidil dose, which would be expected to reduce non vellus hair count 😑

🥊 Clascoterone vs Finasteride for hair loss.

We all know Finasteride is a potent DHT inhibitor, but the potential for systemic absorption makes it unfavourable for some people.

Clascoterone takes a different approach to finasteride. Instead of reducing DHT levels, clascoterone block the signals. This means you still have the same level of DHT in your system.

Clascoterone is in Phase III clinical trial and waiting for FDA approval. I am keen to see more studies comparing Finasteride and Clascoterone.
