From 1 July 2022, all Victorian Primary Care Partnership program funding will transition to Local Public Health Units. Primary Care Partnership functions for the South East Melbourne region will now be delivered through the South East Public Health Unit (SEPHU). This includes all functions previously carried out by Southern Melbourne Primary Care Partnership.

SEPHU looks forward to working with SMP*P stakeholders as they start to incorporate Primary Care Partnership functions within SEPHU's broader public health response.

All SMP*P enquires should be directed to Julie-Anne Coyle (julieanne.coyle@monashhealth.org)
