Brisbane North PHN Helping our community to access the right care, in the right place, at the right time.

Brisbane North PHN supports clinicians and communities in Brisbane’s northern suburbs, Moreton Bay Regional Council and parts of Somerset Regional Council. It covers approximately 4,100 km2 of urban, regional and rural areas, with a population of over 900,000. The Federal Government announced the establishment of 31 Primary Health Networks to replace the national network of Medicare Locals in 2015. The Government selected successful PHNs through an open and competitive process, including the Brisbane North PHN. The key objectives of the Brisbane North PHN are:
• increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes; and
• improving coordination of care to ensure patients receive the right care in the right place at the right time. The PHN will achieve these outcomes by:
• understanding the health care needs of our community through analysis and planning, helping to identify and address service gaps;
• providing practice support services so that GPs are better placed to provide care to patients, keeping them healthy and out of hospital;
• supporting general practices in attaining the highest standards in safety;
• assisting general practices in understanding and making meaningful use of eHealth systems;
• working with other funders to purchase or commission health services for local groups most in need, including patients with complex chronic conditions or mental illness. The PHN supports GPs and other health professionals, and delivers a range of programs. Supporting health care professionals with:
• localised clinical pathways
• electronic referrals
• accreditation and quality standards
• chronic disease management and care coordination
• eHealth records and secure messaging. Managing healthcare and support programs for people in the community including:
• Commonwealth Home Support Program
• Partners in Recovery
• Brisbane MIND, Brisbane MIND Suicide Prevention Program, Brisbane MIND4KiDS
• Team Care Coordination
• Closing the Gap programs. Brisbane North PHN acknowledges Aboriginal and Torres Strait Islanders as the first people of Australia and we pay our respects to the Traditional Custodians on whose land we walk, we work and we live.

🧑‍⚕️The Nurse Re-entry Program is an Australian Government initiative that supports general practices to build workforce capacity by hosting nurses in paid, supervised placements.

Participating practices are supported every step of the way, including:
• matching with an eligible nurse
• practical resources and guidance
• support to smoothly integrate the nurse into your practice team.

The program supports:
• Nurses returning to practice after 5–10 years to re-enter the workforce
• Nurse practitioners transitioning into general practice.

Eligible practices may also receive a practice incentive.

🎉 Brisbane North PHN congratulates Dale Howe who received the Norfolk Island Senior Citizen of the Year award for 2026 🎉

Dale first joined the Brisbane North PHN Community Advisory Committee (CAC) in November 2022 and was recently reappointed for a second term. As a representative of Norfolk Island, Dale ensures her community’s unique perspectives health needs are heard through her active participation on the CAC and other opportunities led by the PHN and their partners.

Dale is a highly valued member of the Brisbane North PHN CAC, and is passionate about addressing barriers to better health on Norfolk Island, particularly improving health literacy of residents.

Older persons residing in Brisbane North access residential care at higher rates than the rest of Queensland or Nationally, with residents living in residential aged care homes (RACHs) often having higher rates of multiple complex conditions.

We know that regular, proactive and continuous general practice care results in better health outcomes for RACH residents. Central to this, is the establishment of strong connections between RACH staff and GPs to facilitate and improve access to quality primary care.

Through our engagement with GPs, primary care stakeholders and the 86 RACHs operating within our region we identified that establishing these connections can be difficult and at times RACHs experience barriers identifying GPs with a special interest in caring for older people.

Enter, Care Provider Connect - a bespoke online portal, codesigned with industry stakeholders intended to address these barriers, streamline and facilitate connection between RACHs and GPs, providing residents with better access to quality primary care.

The portal functions like a virtual ‘positions vacant’ noticeboard where:

📌 GPs can list their availability to provide services to aged care residents.
📌 RACHs can post the support their facility requires.
📌 The GP and RACH can connect via a secure messaging service.

It’s easy to use, improves healthcare access for aged care residents, and builds stronger connections. Ultimately, it delivers access to quality care to aged care residents who need it most.

This portal is the first of its kind to bridge the gap between aged care homes and local GPs and the response has been overwhelmingly positive.

Brisbane North PHN hosted a stall at the recent Kilcoy Multicultural Carnival, showcasing the resources and programs we’ve developed for the region.

The event was a joyful and colourful celebration of the cultural groups and traditions in Kilcoy. It featured traditional dances, cultural performances, and a Welcome to Country and smoking ceremony by Jinibara mob.

Here are some stand outs from our team:

🎶 “The performers sharing their stories and cultural performances with the audience” – Anu, Aged Care team

🫶 Connecting with members of the community and seeing so many healthcare services represented.

The event, hosted by the Somerset Regional Council, was a fantastic opportunity to meet the community and share information about local aged care and mental healthcare resources.

We’ll leave the last word to David from our Priority Communities team. While he appreciated the opportunity to connect with local residents, another moment stands out in his memory – the First Nations dancers inviting the community and political leaders to join them. “That was truly special,” he reflected.

🎉 Congratulations to Jane Schmitt on her appointment as the new Board Chair of Brisbane North PHN, following four years of dedicated service as a Director.

Jane brings extensive experience in senior executive leadership and board governance, which will help guide the PHN’s strategic direction and strengthen its commitment to improving health outcomes across the community.

Her expertise spans advocacy, strategy and policy development, governance, stakeholder engagement, and corporate affairs. With a background in law and governance across health, sport, energy, and education, Jane has a deep understanding of effective board relationships and engagement.

“I am honoured to take on this role and look forward to working with the Board and Executive team to maintain a positive culture that supports collaboration and accountability. Together, we will continue to enhance health and community care for the North Brisbane and Moreton Bay region,” Jane said.

“With over one million people living in our region, it is essential we work closely with stakeholders, government, health providers, and the community to coordinate care—ensuring people receive the right care, in the right place, at the right time. Achieving meaningful outcomes relies on strong relationships, sound governance, and a culture that fosters partnership and innovation.”

Over the past year, 88,500 people received a Brisbane North PHN-commissioned service across the North Brisbane and Moreton Bay region.

The people in our community illustrate the real impact of having accessible, affordable primary care close to home. Here are some of the stories from our 2024–25 Year in Review.

🫶 Nicole, who has lived with persistent pain for 14 years, said her quality of life improved immeasurably after accessing specialist care from the Health Alliance’s SPAN program. The program provided a support network and access to pain specialists who truly understood her situation. ‘The program has given me so much more than I ever believed possible.’

🗣️ Ben, who sits on the Community Advisory Committee, helped create a Lived and Living Experience Lifecycle Kit. He said it’s important to ‘have a voice as to what works, what doesn’t work, and the challenges we’ve had’. He hopes this will pave the way for more representatives from priority groups to share their voice and shape healthcare programs.

🏡 The growing housing crisis has impacted vulnerable populations, with older Australians and people aged 12–25 needing support to find secure housing. We launched a Housing and Homelessness Guide to help care finders support older people, as well as a Specialised Mental Health Service for young people through Brisbane Youth Service. After the first year, 87.5 per cent of these young consumers said they were ‘happy’ or ‘super happy’ with the support.
