SPORTS INJURIES OF THE LOWER LIMB

Often Podiatry is not immediately considered as the first clinicians when it comes to sub-acute ankle sprains, Lisfranc's, turf toe, plantar plate rupture(s) stress fractures or shin splints. In children Osgood Schlatter's or Sever's disease (heel pain) or Iselin's disease

However, we have very comprehensive biomechanical and anatomical knowledge that enables us to conservatively treat, mentioning above only a few of the many injuries that can be acquired whilst undertaking sport or physical activity.

The common cause of injury is poor conditioning/ strength or muscular imbalances leading to tissue stress-strain overload = INJURY.

Inversion (sprained ankle) as an example:
Proven risk factors
- Lack of strength and stability related to the ankle.
- Lack of, or extreme flexibility, in the ankle joint.
- Poor balance.
- Sudden change in direction (acceleration or deceleration).
- Increasing age of player

Suspected risk factors
- Poor condition of the playing surface
- Inappropriate, inadequate, or no warm-up.
- Wearing inappropriate footwear for the activity.

Another very important, but commonly under emphasised is specific footwear recommendations for your foot type and activity being undertaken.... pronation is a necessary motion in our feet for shock absorption, without it or wearing footwear that restrict this motion too much can lead to musculoskeletal pathology.

Podiatry and foot care supports are supports to help you treat and manage conditions that affect your feet, ankles and legs. The Australian health system may provide podiatry and foot care supports. We can only fund these supports if they’re related to your disability.

We may fund podiatry and foot care supports if you need them because of the things you can and can’t do because of your disability. And they help you undertake activities of daily living. These supports might include specialised shoes or funding to treat your foot problems. Podiatry and foot care are especially important if you have a disability that affects sensation to your feet.

Do I need a referral to see a Podiatrist?

If you are a private client you do not require a Doctor's referral and can self refer.

If you are attending under the following initiates, then you will require a Doctor's referral, please see below...

1. Medicare Chronic Disease Management Plan (10962 referral).
A team care arrangement if firstly completed whereby you discuss what Allied Health care professionals/ services will most benefit you in receiving allied health management if you have a chronic health condition or complex comorbidities.

For example: You are allocated up to 5 visits per Calander year, this means you may nominate 4 x visits for Podiatry and 1 x to Physiotherapy.

You agree once discussed with your Doctor to share the total 5 visits for multidisciplinary management.

This is not age limited, all ages at the discretion of your Doctor are eligible.

At Knee to Toe Podiatry we bulk bill consultations with 'no GAP or out of pocket' expense only for the consultation.

Any additional Podiatric consumables such as orthotics incur full cost recovery by the client. The cost of any proposed treatment is discussed with you so you can make an informed decision.

Feet & choosing footwear...

There is an enormous amount of consumer choice and it can be a little confusing as to the best shoe for the activities you will be undertaking or if you have feet pathology when wearing your shoes.

This is very important for children and teens, when they are going through fast developmental growth spurts or if you wear orthotics or have feet deformity/ pain.

Runners are not just athletic footwear. This style of footwear contain advanced evolving technology, cross trainers, road vs trail running shoes, in tempo runs and sport specific footwear for tennis, netball, basketball and NRL vs AFL just to mention a few.

There are neutral, mild support, stability straight vs curved last rail guards and carbon plates for energy return.

Chilblains, also known as Pernio

A medical condition in which damage occurs to capillary beds in the skin, most often in the hands or feet, when blood perfuses into the nearby tissue, resulting in redness, itching, inflammation, and possibly blisters.

It occurs most frequently when predisposed individuals, predominantly women, are exposed to cold and humidity.

Temperature-related chilblains can be prevented by keeping the feet and hands warm in cold weather and avoiding exposing these areas to extreme temperature changes.

First-line treatment includes avoiding cold, damp environments and wearing warm socks or thermal insulative anti-shear shoe liners.

Chilblains can be idiopathic (spontaneous and unrelated to another disease), but similar symptoms may also be a manifestation of another systemic medical condition that must be investigated. Related medical conditions include Raynaud syndrome, erythromelalgia, frostbite, and trench foot, as well as connective tissue diseases such as lupus or vasculitis.

The areas most affected are the toes, fingers, earlobes & nose.

SIGNS & SYMPTOMS
Blistering of affected area
Burning and itching sensation in extremities
Dermatitis in extremities
Ulceration (severe cases only)
Erythema (blanchable redness of the skin)
Pain in affected area
Skin discoloration, red to dark blue

There are pharmaceutical medications topical &/ or oral that can be prescribed in recurring and serious cases. A medication history and review is required to ensure there are no adverse interactions if you are taking other prescribed medications.
