Today is Remembrance Day on the eleventh day of the eleventh month, a day to honour the sacrifice of those who have died serving Australia in wars, conflicts, and peacekeeping operations.

It is an important time for all Australians to acknowledge the courage, contribution and service of our armed forces, both past and present.

On this day at 11am, we pause for one minute of silence to remember their bravery, their stories, and their legacy.

Our Clinical Care Coordinator, Aslin, recently supported one of our participants, Theresa, to join the Hume Council’s weekly community coffee, chat, and lunch group at the local neighbourhood house. Aslin also attended the centre with Theresa and her support worker to ensure she felt welcomed and comfortable. As a result, Theresa has now gained the motivation and confidence to attend this group on a weekly basis.

At Gratitude Home Care, we go above and beyond to create meaningful opportunities for our clients to engage with their community. These activities help reduce social isolation, build confidence, and support overall wellbeing.

Most importantly, we take the time to understand each person’s interests, ensuring the activities we organise are enjoyable, meaningful, and empowering.

Support at Home: Unspent Funds

If you already have a Home Care Package (HCP) before the transition to the Support at Home (SAH) program on 1 November 2025, you will keep all of your unspent funds, no matter how much you have. Those funds will carry over and remain available for approved supports, equipment, and home modifications.

The $1,000 unspent funds limit applies only to new participants who enter the Support at Home program after it begins on 1 November 2025.

There is no earlier cutoff date (like November 2024) for this rule, it’s based purely on whether you were already an HCP recipient before the SAH start date.

Participant Appreciation - Souad

During one of our Clinical Care department’s regular check-ins, Souad shared how much she appreciates the support workers who assist her and expressed gratitude for the regular face-to-face visits from our team. She also noted how much she values that we’re always able to assign support workers who match her preferences.

During the visit, Aslin, one of our Clinical Care Coordinators, had the opportunity to discuss future bathroom renovations that will make shower accessibility easier, as well as talk through the upcoming Support at Home Program launching on November 1.

Some participants, like Souad, prefer face-to-face communication over phone calls, so we make sure to visit regularly to discuss important updates and plans in person.

At Gratitude, our Clinical Care team provides regular check-ins and ongoing care management to ensure every participant’s support matches their needs. We stay connected, update care plans as needs change, and support every participant with kindness and respect.

Yesterday we exhibited at the 2025 CARE Melton Expo! Great teamwork made the whole event seamless.

Thank you to the Melton City Council team for hosting such a fantastic event and special thanks to Deputy Mayor Lara and Councillors Brandi and Kathy for the warm welcome and for creating such a valuable community event.

We were thrilled to connect with so many locals, seniors, carers, people living with disability, and those from culturally and linguistically diverse backgrounds. The day was not only inspiring but also a huge success, with many meaningful conversations and new connections.

The Australian Government’s 2024 Aged Care Residents’ Experience Survey has found improvements in residential aged care, with responses from more than 36,000 residents across 2,603 homes.
The survey showed that 95% of residents feel safe in their homes and believe staff are kind and caring. A total of 91% said they have a say in their daily activities, a 10% increase since 2022. Around 88% reported being encouraged to do things for themselves, up 7% from 2022, while 88% said they would recommend their aged care home, a 3% rise compared with 2023. Satisfaction with food remained steady at 70%.

New Food and Nutrition Standards will come into effect in November 2025, aiming to improve the quality and variety of meals in aged care. The survey findings also form part of each home’s Overall Star Rating, which is published on the My Aged Care website.
