We're Hiring Aged or Disability Carer - South Hedland / Pilbara 💛
Essential Nursing & Disability Services is in need of an Aged or Disability Carer for our South Hedland/Pilbara operations. Flexible hours available. 2-3 years of relevant work experience required for this role. Expected salary: $76,515.
DUTIES AND RESPONSIBILITIES
• Assessing clients’ needs to plan, develop, and implement tailored support programs.
• Conducting interviews to understand the nature and extent of clients’ difficulties.
• Monitoring client progress and providing reports to track development and support effectiveness.
• Evaluating community needs for health, welfare, housing, employment, and training services to allocate resources effectively.
• Assist clients with a range of appropriate community access and companionship (recreational, pre-vocational, education and social) aimed at increasing the individual’s skills and access to and participation in the community
• Assist people with mental health disability at their homes or at SDA accommodations
• Maintain a high level of service for all individual’s and ensure the health, wellbeing and safety of all people supported and staff is maintained at all times
• Administer medication as required and according to service policy and procedure
• Uphold the Code of Conduct and Confidentiality Statement in all decisions, behaviours and actions and comply with company’s policies procedures and workplace instructions ensuring best practice in service delivery is achieved
• Assisting young people with social, emotional, and mental health issues, providing support as per company’s policies procedures.
• Preparing submissions for funding, resources, and reports for government bodies and agencies as per the company’s policies and procedures.
• Working across service units to provide community access and support to individuals with intellectual, physical, social, and emotional disabilities.
Support the planning and participation in recreational and lifestyle activities that align with the client’s interests and personal goals.
· Run essential errands such as grocery shopping, prescription collection, and bill payments as required.
· Accurately record daily care activities, changes in client condition, and any incidents, in line with organisational procedures and privacy standards.
· Collaborate with family members, support coordinators, allied health professionals, and other care team members to provide consistent, person-centred care.
· Be available to provide overnight or live-in support when necessary, depending on the client’s needs and care arrangements.
