“A New Clinical Framework for Tinnitus Care
-Built from Practice” — By Mini Gupta our own Tinnitus & Sound Sensitivity Specialist.

Tinnitus care is not limited by lack of research. It is limited by the way we continue to think about it.

We have spent years trying to manage tinnitus as if it were a single, stable problem.

It is not!

In clinical reality, tinnitus is a dynamic, system driven process, shifting between auditory, limbic, autonomic, monitoring, cognitive, somatosensory and physiological influences. And yet, our frameworks have not evolved to reflect this.

That gap is where outcomes are being lost.

Over the past decade, my work has been focused on understanding these patterns at a level that is clinically usable, not just theoretically sound.

What has emerged is a structured, practical framework that changes how we assess, prioritise and intervene.

I will be presenting this framework for the first time in my inaugural course:

Mastering Tinnitus Clinical Skills – Level 1
6th and 7th June, Melbourne.

This is not about attending another course.

It is about building something together that reflects how tinnitus actually behaves in real patients, with the intention of taking it forward for publication.

Not for everyone!

Only for clinicians who are willing to challenge what they have been taught and take responsibility for how this field evolves next!

This is where a new clinical narrative will begin!!

The power of TRT , when it is done right and followed fully

This 18 year old came in with severe hyperacusis. Not just sound sensitivity but real functional impact. School had dropped to 1 day a week. Social environments were not possible. Sound was simply too much for his system.

Tinnitus was present but not the main issue. TFI was 25 at baseline. Sound sensitivity was the bigger problem.

We completed APD testing as well and no evidence of APD.

We started proper TRT - clear directive counselling , individually programmed sound therapy for each ear , sleep sound therapy and sound enrichment .

6 months on : note the difference in UCLs. This is not just numbers improving. This is the auditory system coming out of protection mode achieving regulation of central gain mechanisms.

TFI is now 10 from 25…and this is a bonus.

And you can see it in his life.
School is now 4.5 days a week.
First ever birthday outing done.
Sleepover with friends completed, without needing to be picked up.

One more thing that cannot be ignored - his mother. Driving almost 3 hours to and from clinic, sometimes twice a month. And a teenager who showed up and stayed consistent.

TRT works. But only when it is done properly… and followed properly.

We are only halfway. Next 6 months will be crucial as we move to next level of sound desensitisation with more targeted sounds.

CBT will be an essential component too- to remove the learnt cognitive distortions around sound in general.

Goal: a live music concert next year.
Not as a challenge. Just as life returning back to where it should be for a healthy social human.

This is what life changing treatment outcomes look like

Autistic children may experience:

Hyperacusis
• ordinary sounds feel too loud or physically uncomfortable
• the body reacts with tension or distress

Misophonia
• specific sounds (often human or repetitive) trigger strong emotional reactions
• reactions feel immediate and overwhelming

These are forms of decreased sound tolerance, not signs of ear damage or emotional immaturity.

The Science of Feeling Safe With Sound👂

The auditory system didn’t evolve just to help us hear better. It evolved to help us feel safe.It is constantly scanning the environment, checking if everything around us is okay.

When the auditory system is healthy, the brain relaxes. Sound feels normal. It does not demand attention.

When tinnitus or sound sensitivity begins, it is rarely just about the sound. It is about what that sound now means to the nervous system.

You feel irritable. Tired. On edge.Even when you cannot explain why.

Sound shapes how safe we feel in a space. When sound feels safe, the body softens. When it does not, the whole system stays alert.

Our work is not only about hearing tests. It is about helping people feel safe in their own sound world again.

Grateful first. Always.
My team and I sincerely thank our local and interstate audiology colleagues for their referrals, trust, and for working with us as part of a shared tinnitus care journey.

Today we completed the second tinnitus therapy session for a patient with hearing loss who was fitted with ITC hearing aids two weeks ago by another local audiologist. The hearing aids helped his tinnitus during the day, but at night and when the aids were out the tinnitus became catastrophic.

Today was the first time we used the Noise–Meaning–Feeling–Action framework.
In one session, the patient mapped how his tinnitus felt last week versus today. Seeing that contrast on paper and crossing out the earlier column with his daughter became therapy in itself.

The sheet shared here is from today’s session and shows a clear shift in tinnitus perception, meaning, and emotional response.

At the end of the session, the patient said:

“It feels like someone has done an operation on me and taken out the suffering from the tinnitus.”

On the back of the sheet, he wrote in Greek:

“Words are too poor to express what I am feeling right now. I and the ‘guest’(tinnitus) have found peace again.”

Effective tinnitus treatment requires two things: optimising hearing function and calming the nervous system both are equally important.

For audiologists: we are always happy to support your tinnitus patients, whether after hearing aid fitting or after a hearing assessment in cases of normal hearing. Patients are discharged back to you for ongoing care.

Tinnitus outcomes are strongest when we work together.

More than ten people have reached out today, anxious after the concert -describing a ringing or buzzing sound in their ears that hasn’t stopped since.

That sound is called tinnitus and after loud music exposure, it’s often temporary.It usually settles within 48 hours as the auditory system recovers.

To ease the discomfort, try sound enrichment — play gentle nature sounds (like ocean waves or a soft waterfall) at a pleasant, low level in the background. This helps the auditory system stay calm and supported while it settles.
