Hunter Primary Care is a not-for-profit organisation located in Newcastle NSW. For over 30 years we have been supporting people to live a healthy life by delivering a range of health and wellbeing services to the Newcastle and Hunter community. Our services include -
• Aboriginal & Torres Strait Islander Health Services including care coordination and chronic disease management, mental health care coordination and psychology services
• Aged Care services including Aged Care Emergency (ACE), Aged Care Nurse Practitioner, Aged Care Clinical Placements Program and psychology services
• Allied Health services including Priority Allied Health, Multidisciplinary Lifestyle Community Groups and Coordinated Respiratory Support
• GP Access After Hours
• Mental Health Services including headspace Newcastle, Commonwealth Psychosocial Support and psychology services
• NDIS Services including Group Programs, Level 3 Specialist Support Coordination, Positive Behaviour Support, Psychology and Allied Health including dietitians, occupational therapists, exercise physiologists and social workers. Connect.

📢 NDIS Update 📢
A recent Federal Court ruling has widened NDIS eligibility for some participants, particularly those with early access to aged care packages.
The Court clarified that eligibility should be based on the likelihood of requiring lifelong disability supports, not whether other systems (such as aged care) can partially meet current needs.
This outcome may impact eligibility assessments, appeals and pathways for older participants and providers working at the intersection of NDIS and aged care.

Our NDIS team had a great time at the Maitland Ready-Set-Connect event this morning. Ready-Set-Connect is a fun and interactive networking day bringing together service providers, people with disability, parents and carers, support coordinators and Local Area Coordinators.
Events like this are all about connection - sharing information, learning about local services and supporting people to exercise choice and control. We loved meeting so many members of the community and having meaningful conversations along the way.
Thank you to everyone who came along and helped make the day such a positive one.

We’re proud to celebrate the incredible creative work of Emma, Regional Coordinator for our Aged Care Clinical Placements Program, who recently completed an artist residency at Lighthouse Arts Newcastle, based at Whibayganba.
Immersed in creativity and mindfulness on Awabakal land, Emma used the residency as a space to explore new ideas, connect with community, and draw inspiration from the surrounding coastal environment.
Emma’s cyanotype prints and photography are inspired by her love of the ocean, lighthouse landscapes, and Aboriginal culture, reflecting a strong connection to the natural environment and the rich cultural history of the land.
We love seeing the diverse talents and passions of our people shine, both on and off the job.

Today we acknowledge Survival Day. For many Aboriginal and Torres Strait Islander peoples, this day marks the ongoing survival of the world’s oldest continuing cultures despite the impacts of colonisation. It is also a day of reflection and mourning, acknowledging loss and the intergenerational impacts that continue to be felt.
At Hunter Primary Care, we acknowledge Survival Day as an opportunity to listen, learn and stand alongside Aboriginal and Torres Strait Islander peoples, with respect for culture, strength and resilience.
We acknowledge that people experience this day in different ways, and encourage our community to take a moment to reflect on the significance of this day and to show care and consideration for those who may find it difficult.

If you previously used the My Health App, you can now find the same support under the new name, 1800MEDICARE.
