Hunter Primary Care is a not-for-profit organisation located in Newcastle NSW. For over 30 years we have been supporting people to live a healthy life by delivering a range of health and wellbeing services to the Newcastle and Hunter community. Our services include -
• Aboriginal & Torres Strait Islander Health Services including care coordination and chronic disease management, mental health care coordination and psychology services
• Aged Care services including Aged Care Emergency (ACE), Aged Care Nurse Practitioner, Aged Care Clinical Placements Program and psychology services
• Allied Health services including Priority Allied Health, Multidisciplinary Lifestyle Community Groups and Coordinated Respiratory Support
• GP Access After Hours
• Mental Health Services including headspace Newcastle, Commonwealth Psychosocial Support and psychology services
• NDIS Services including Group Programs, Level 3 Specialist Support Coordination, Positive Behaviour Support, Psychology and Allied Health including dietitians, occupational therapists, exercise physiologists and social workers. Connect.

Zac is a senior behaviour support practitioner with a double degree in Psychology and Criminology and a Graduate Certificate in Applied Behaviour Analysis. He supports children and adults with intellectual disability, autism, epilepsy, and complex behavioural needs through evidence based assessment, positive behaviour support, and trauma informed practice. He works collaboratively with families, schools, and services to build team capacity and provide clear, sustainable support that enhances quality of life for the person.

Today is Close the Gap Day, a national reminder that achieving health equity for Aboriginal and Torres Strait Islander peoples requires sustained action, partnership and accountability.
Hunter Primary Care is marking the day by launching our Stretch Reconciliation Action Plan (RAP) 2026–2029, reaffirming our commitment to reconciliation and culturally safe healthcare.
Our new RAP strengthens engagement with Aboriginal and Torres Strait Islander stakeholders, embeds co-design in service development and reinforces our role in contributing to Closing the Gap in health outcomes within our region.

This week marks Neurodiversity Celebration Week, a time to recognise and celebrate the diversity of how people think, learn and experience the world.
At Hunter Primary Care, many of the individuals and families we support are neurodiverse. Through our Psychological Assessments service, our clinicians work alongside clients to better understand their unique strengths, challenges and support needs. Assessments can be an important step in helping people access the right supports at school, work and in everyday life, and in fostering greater understanding for individuals and their families.
During Neurodiversity Celebration Week, we acknowledge and celebrate the many different ways people experience and navigate the world, and the importance of creating communities that recognise and support those differences.
