About the Role
The purpose of the Support Coordinator, is to assist NDIS participants in the support coordination and implementation of all supports in their funded plan, including informal, mainstream and community supports, as well as funded supports.
This position is initially being offered on a 6 month contract basis, which will eventually turn into a permanent position
Key Responsibilities
• Provide coordination / support connection to participants and their families / carers, persons responsible.
• Support the customer to implement their NDIS plan, including assistance to identify and select providers
• Monitoring plan outcomes & plan expenditure to ensure the plan remains within the budget
• Strengthen and enhance the participant and / or carer’s abilities to self-direct their own supports and participate in the community
• Ensure mainstream services meet their obligations (i.e. housing, education, justice, health)
• Ensure clear, accurate and timely record keeping including making information accessible to the participant
• Capture any required data for the organisation and the Agency
• Provide the NDIA, and the participant on request, reports on progress and achievements within the agreed reporting frequency
Skills & Experience
• Relevant Tertiary qualifications in disability or related discipline
• Demonstrated experience within disability services or related services (i.e. Disability Employment Services), including supporting people with disability to develop positive relationships with a broad range of internal and external stakeholders
• Demonstrated experience in facilitating personal planning processes
• Demonstrated application of the objectives and principles of the National Standards for Disability Services
• A professional and positive outlook about supporting people with disabilities and commitment to supporting and promoting the individual needs, skills, abilities and personal goals of people with disabilities.
• Commitment to the values of dignity, respect, inclusion, empowerment, confidentiality and strength-based practice
• Excellent case note reporting and record management
• Demonstrated computer skills in Microsoft Word and Excel
• Knowledge of the NDIS and price guide
Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
