Dizzy? Has your world ever been spinning?
DR ALFIE ARCIDIACONO'S PRACTICE CASE STUDY
Benign paroxysmal positional vertigo (BPPV) is a disorder arising from a problem in the inner ear. People can experience dizziness, a spinning sensation (vertigo), lightheadedness, unsteadiness, loss of balance and nausea.
PATIENT STORY:
The patient has surgery 15 days ago and the following morning could hardly walk feeling disorientated. In surgery, whilst great care is taken certain movements of the head may potentiate the shift in little crystals in the inner that cause concerning disorientation and nausea. After an array of medical scans, the good news there was nothing life-threatening. On the advice of her GP, she consulted with me for help.
DON'T WAKE THE SLEEPING GIANT
With a practice specialising in some complex neurological complaints, our treatment was to include a series of head movements that shift particles in the ears to help carefully reposition these crystals. However, despite a thorough clinical examination repeated testing was unremarkable, so we gave this lady some advice on how to proceed and consult with us if things did not improve, as if we did something, we may have risked 'waking a sleeping giant' despite the patient desperate for help.
CLINICAL OUTCOME
This case study is a reminder that sometimes the best treatment is NO treatment, as in actual fact she may have been naturally resolving her problem anyway. To only interfere with this process, may have meant we made things worse. However, the technique we use in practice for such patients who are positive can be very good in many cases, sometimes needing repeated treatment.

Mum and I went straight to Mona Vale beach for a and a walk. The more I moved around the more confident I became and I had no dizziness but did get a bit fatigued which I guess is to be expected as I've been so sedentary lately. I'm slowly going to get more mobile over the next few days with the view of returning to work next week."
On our further follow up she positively replied: "I feel 100% today - best I've felt in weeks!"
