Dizzy? Has your world ever been spinning?

DR ALFIE ARCIDIACONO'S PRACTICE CASE STUDY

Benign paroxysmal positional vertigo (BPPV) is a disorder arising from a problem in the inner ear. People can experience dizziness, a spinning sensation (vertigo), lightheadedness, unsteadiness, loss of balance and nausea.

PATIENT STORY:
The patient has surgery 15 days ago and the following morning could hardly walk feeling disorientated. In surgery, whilst great care is taken certain movements of the head may potentiate the shift in little crystals in the inner that cause concerning disorientation and nausea. After an array of medical scans, the good news there was nothing life-threatening. On the advice of her GP, she consulted with me for help.

DON'T WAKE THE SLEEPING GIANT
With a practice specialising in some complex neurological complaints, our treatment was to include a series of head movements that shift particles in the ears to help carefully reposition these crystals. However, despite a thorough clinical examination repeated testing was unremarkable, so we gave this lady some advice on how to proceed and consult with us if things did not improve, as if we did something, we may have risked 'waking a sleeping giant' despite the patient desperate for help.

CLINICAL OUTCOME
This case study is a reminder that sometimes the best treatment is NO treatment, as in actual fact she may have been naturally resolving her problem anyway. To only interfere with this process, may have meant we made things worse. However, the technique we use in practice for such patients who are positive can be very good in many cases, sometimes needing repeated treatment.

"Thanks so much for your time and all of your follow up. I was really impressed with the time you took today to explain everything and to reassure me. Mum and I went straight to Mona Vale beach for a and a walk. The more I moved around the more confident I became and I had no dizziness but did get a bit fatigued which I guess is to be expected as I've been so sedentary lately. I'm slowly going to get more mobile over the next few days with the view of returning to work next week."

On our further follow up she positively replied: "I feel 100% today - best I've felt in weeks!"

Shoulder Blow Out?

We often take the health of our joints for granted until they start playing up, and just like a wheel alignment, everything seems to work OK until you start losing tread. If you let this continue, the thin tyres my blow out, causing an accident that could be catastrophic. But probably, if you are like most people and this happened to your shoulder you would say "but this injury just happened, I've never had a problem before!"

Shoulder Knock-On Effect

This is exactly the problem. It is often a little injury that may appear to resolve pain wise, but you are left with the subtle misaligned shoulder dysfunction. Over weeks, months and years you now develop a 'structural' change where the shoulder and surrounding soft tissues (rotator cuff), ligaments, tendons and more may be damaged, and sometimes to a point of irreversible damage if ignored. Hopefully, this is not the case if you have suffered a shoulder injury. This is why a comprehensive shoulder and spinal assessment can be priceless in catching early dysfunction and circumvent a more chronic shoulder problem, that will often have a 'knock-on effect' on the neck, mid back and often the whole arm itself.

Most Common Shoulder Complaints

SIX TIPS TO HELPING STIFF & ACHY JOINTS THIS WINTER!

Brrrrrrrr! Looks like winter has kicked in. This is a common time that those sleepy joints and stiff muscles will after wake up, rather than go into winter hibernation. It's often a reminder that we need to ensure we are taking greater care of our musculoskeletal system throughout our years to ensure we can 'age gracefully'.

This is the time of year we see a lot of our patients return to the practice for care, sometimes with an acute flare-up! Here are some tips to help you with your stiffness, aches and pains this winter:

1. Stay warm. A cold polar wind can wake up some of those latent or sleepy trigger points, and you wake up with a stiff neck or worse still severe spasm in your neck where you can hardly move! Wear a turtle-neck top, hoodie and/or scarf! Where a singlet to keep your back warm.

2. Use a mild heat pack on those stiff joint for 20min. Remember, soft tissue behaves like a piece of metal. If you heat it up, it becomes more ductile, allowing great flexibility. Cold stiffens things up.

3. Keep active, despite maybe not feeling like it. This ensures the joints and muscles have a healthy active blood supply bring nutrients into the joints and flushes the waste by-products of inflammation that stiffen you up and cause pain.

4. If you are going to get active with sport, house-work or any other physical chore, consider doing some pre-activity stretching. This will prevent an unnecessary injury.

6. Get adjusted again if it's been a while.
