Hi there
Aprochc Health services, are a Fee-for-service provider, which means we can support participants who are NDIS plan managed or self-managed. We cover all of the Brisbane areas.
We currently have capacity to support 2 participants and their families.
We provide in-home assistance with daily support:
* Personal Care:
Support and supervision of daily personal living tasks and private hygiene and toileting, along with dressing and maintaining your personal appearance. Personal care may include Bathing and Showering, including bed-baths. Applying lotions and creams as required.
* Domestic Chores:
We can support people to maintain their independence within their own home with a wide range of services, such as cleaning, meal preparation, ironing, Laundry, companionship and a range of other services to meet individual participants needs.
* Community Access:
Support participants to physically access environments external to the home. Making doctors' appointments, or any regular appointments or shopping, to walks in the park.
* Community Activities of choice:
To support participants, needs to meet goals relating to participating in community, social or recreational activities. Also supports if you are studying on campus, attending a concert or any supporting event.
