Live Your Best Life Support Services An Independent Support Service offering support to NDIS Participants to Live their Best Life Services are available within the Mid North Coast area.

I am a mature aged female who is an independent support worker living with a disability. I became a below knee amputee in 2017 and now wear a prosthetic limb. I am very mobile, capable and an independent person who believes our disability does not define who we are, it is a mere extension of ourselves. I am a former dental assistant in a local Taree dental surgery where I worked for over 15 years more, recently a Local Area Coordinator (LAC) with Uniting in partners with the NDIS. I am offering support to empower participants and build their capacity to be as capable, independent and knowledgeable about their disability needs. I'm offering support in:

*Accessing the community and social participation (e.g.: shopping, appointments, community and social participation).

*Capacity building skills (e.g.: meal preparation, budgeting, planning, basic cleaning skills and travel training).

*Support Connection for those participants that need to find and connect with suitable providers, understand their NDIS Plan and Funding.

*Aged Care Services (e.g.: companionship, appointments, services through Mable Platform).

All supports are individualised to meet your particular needs. Participants plans must be Self-Managed or Plan Managed.

Meet and greet appointments are highly encouraged to discuss your needs and to see if I am a good fit for you. I have all relevant documentation and insurance.
