🌟 Last Outreach Trip for 2024 🌟

Sunny Side Up has one last CQ outreach trip for the year on Dec 12, 13 & 14. We only have 4 spots remaining for the below occupational therapy services with Senior OT, Ellie:

Functional Capacity Assessments (have a completed report by Christmas)
AT Assessments
NDIS Access Requests
Sensory Assessments & Home Programs
Initial OT Assessments
ILO, SIL & SDA Applications

Please make any referrals by end of day December 5 so that we can finalise travel arrangements and appointment times!
