Fingertip injuries are common, and there are many ways to treat them. Options include shortening the finger, dressing it until it heals over, skin grafting, advancement flaps (such as a VY) and neurovascular island flaps. This patient had a partial amputation of his ring fingertip at work. I elected to perform a neurovascular island flap to preserve the finger's length, decrease recovery time (as is required with dressings), and keep the tip's sensation. A VY wasn't chosen in this case as it's challenging to move the resultant flap enough to cover the defect.

The neurovascular island flap shown here was described by Segmuller in 1976 and required raising a patch of skin on one side of the digit's neurovascular bundle. It can be moved up to 2 centimetres once raised, so is very versatile, and can be done on both sides of the finger for large defects. The photos here show the progress over two months. Healing is relatively straightforward, but hand therapy is required throughout for dressings and to encourage range of motion.

K-wires are frequently used in hand surgery to hold fractures together as they heal. They are removed in the office without anaesthetic once the fracture has healed (generally around four weeks). Removal doesn't cause pain.

Kirschner wires (K-wires) were first used by Martin Kirschner in 1909, and are one of the most versatile instruments in Orthopaedics. K-wires are smooth pointed wires made of stainless steel and mainly used to fix broken bone fragments in place. We can temporarily use them while reducing a fracture for plating or use them as definitive fixation. They are also used as retractors, levers, strengthening in cement spacers, to make external fixators, and just about anything imaginable.

I use them a lot in hand fractures, and this shows one of the most common uses, to hold a distal phalanx stable before repairing a nail bed. Thinking about the removal of wires often causes anxiety in patients, but it is quick and painless.

Today we had the pleasure of welcoming Dr Allison Hallett for a visitation to see shoulder arthroplasty. Dr Hallett is an experienced shoulder surgeon originally from the UK, but now working in Bundaberg.
