Critical Analysis of Trigger Point Constructs in Clinical Trials Commentary on Todri et al., 2025—Methodological Considerations and Pain Science Foundations - The recent technique-based clinical trial conducted by Todri et al. (2025) has been both methodologically and categorically praised for its contribution to addressing gaps in the current evidence base related to trigger point techniques in students with cervical pain (Aphale et al., 2025). Setting the osteopathic research agenda: consensus, context, controversy, and capacity - The osteopathic profession achieved something unprecedented: 2229 individuals from 42 countries provided their views on research priorities through the European Research Priorities for Osteopathic Care (PROCare) project - published in BMJ Open in October 2025 [1]. Not only do the results have implications for research projects in the coming years, as members of the osteopathic community, we have also learnt a lot about ourselves in the process.

Response to commentary to the Editor regarding “Immediate effects of three different upper trapezius trigger point techniques on pain intensity and pressure threshold in students with cervical pain: a randomized clinical trial” (Int J Osteopath Med. 2025;58:100777) - This response letter addresses methodological and interpretative issues raised in a recent commentary concerning our randomized clinical trial comparing three upper trapezius trigger point techniques (Jones strain–counterstrain, Lewit post-isometric relaxation, and Chaitow muscle energy technique) with a sham intervention in students with cervical pain. We welcome the constructive critique and provide clarifications regarding the prespecified primary estimand, statistical modeling, clinical interpretation, sham credibility, and generalizability.

Comment on “Efficacy of myofascial unwinding and myofascial release on somatic symptoms in patients with non-specific chronic low back pain – A study protocol for a single-blinded randomized clinical trial” - We read the recently published protocol evaluating myofascial unwinding plus myofascial release versus conventional physiotherapy for non-specific chronic low back pain with somatic symptoms. The trial plans a two-arm randomised design (n=30), four weeks of treatment, and PHQ-15 as the primary endpoint, with numerical pain rating and Q-LES-Q-SF as secondary outcomes [1]. This is a relevant question and the protocol would benefit from a few clarifications before results are reported.

The pragmatic focus on commonly taught techniques, use of standardized outcomes (pain intensity, pressure pain threshold, and cervical range of motion), and inclusion of a sham comparator are notable strengths.

Comment on “Immediate effects of three different upper trapezius trigger point techniques on pain intensity and pressure threshold in students with cervical pain: a randomized clinical trial” - We read with great interest the recent randomized clinical trial by Todri et al. in the International Journal of Osteopathic Medicine [1]. This study provides a valuable contribution to the field of manual therapy through its rigorous, well-blinded design, comparing three established myofascial trigger point (MTrP) techniques – Chaitow, Lewit, and Jones – against a sham intervention. The inclusion of a sham treatment group is particularly commendable, as it helps control for non-specific treatment effects and enhances the study's internal validity.
